Started in 1988, Hospice Toronto is Toronto’s first not for profit, community and volunteer-based, in-home hospice palliative care program. By providing and advancing compassionate care and leadership in the volunteer-based, home hospice community, Hospice Toronto has become a recognized leader in providing high-quality, hospice care and support for people through all phases of their life-threatening illness, and their loved ones, at no charge. We provide support through our In Home, Bereavement and Children's Support Programs; Expressive Arts Therapy; Young Carers Program; and Advocacy & Consultation.

Hospice Toronto is proud to offer Creating Caring Communities (CCC) as one of our key programs supporting people living with life-threatening illnesses.

CCC works closely with residents and local agencies in St. James Town to ensure individuals can receive compassionate, culturally sensitive support right at home. By training and connecting community members, this program helps neighbours care for one another, navigate the health care system, and access hospice palliative care services when they need them most.

Since 2008, CCC has grown through strong community leadership and word-of-mouth connections. With funding from the Government of Canada, this model is now expanding to more regions across Ontario and Nova Scotia, helping even more communities build resilience and support their most vulnerable members.

Today is National Grief and Bereavement Day, recognized each year on the third Tuesday of November. Hospice Toronto, alongside Canadian Hospice Palliative Care AssociationCare, is honouring this day by raising awareness about grief and the importance of supporting those who are experiencing it.

Grief touches every one of us. Yet many people feel unprepared for how long it lasts, how deeply it affects them, or how hard it can be to talk about. Even with the best intentions, loved ones may not know how to offer support — leaving many people feeling alone or misunderstood.

This year, we’re joining the call to help close Canada’s grief literacy gap. When we learn how to talk about grief with compassion and understanding, we make space for healing.

Hospice Toronto is here to help. Our Bereavement Support Program provides a caring, compassionate space for family and friends to talk openly about their feelings and experiences. We offer:

• Short-term one-on-one sessions with a Client Services Coordinator
• Peer-based Bereavement Support Groups
• Volunteer support calls and visits
• Referrals to community bereavement services when needed

Our trained volunteers and staff help people navigate the early stages of grief, share memories and emotions, and find support in a safe, understanding environment.

Let’s work together to ensure no one has to navigate loss on their own. 💙

We’re reflecting on an incredible Hike for Hospice, a day filled with joy, connection, and community spirit!

Together with our partners, Philip Aziz Centre / Emily's House for Hospice Care, and The Salvation Army Toronto Grace Health Centre Palliative Care – we raised over $78,000 in support of hospice and palliative care programs across Toronto.

With support from Employment and Social Development Canada, Hospice Toronto is proud to serve as the central agency of a collective impact seniors project that brings together partners to provide $3.2M in support for over 400 seniors. Collaborators include Community Family Services Ontario, Essential Partners in Care (EPIC), and the Diversity Institute. 💛
