Dear Patients, I am writing to inform you that I am closing my practice of family medicine at the Leaside Health Center as of October 22nd, 2022. It has been my pleasure to serve as your physician and I want to thank you for the trust and confidence you have placed in me.
It is advisable that you find a new physician for future care as soon as possible as there is no physician at the Leaside Health Center who will be taking over my patients. For your convenience, I have included a list of local physicains in a letter mailed to you. You may also contact Healthcare Connect at 1.800.445.1822 for help with finding physicians accepting new patients near you.
After securing a new family physician, please inform all health care professionals (emergency departments, specialists, labs, imaging centers, hospitals and walk-in clinics) of the name of your new physician. This will ensure that reports and results will be sent to your new Family Doctor for review and follow-up. All outstanding correspondences for lab work, imaging studies, etc., given by this office must be completed by the end of October 21st, 2022 for results to be received and reviewed.
I have made arrangements with DOCUdavit Solutions for the storage and management of all patient’s medical records. DOCUdavit Solutions will ensure the security and privacy of your medical records and will provide your new family physician with an electronic or paper copy of your file upon your request. The charge ranges between $30-95 depending on the length of your file, plus applicable taxes and shipping charges.
I would recommend that you obtain a copy of your complete record for review with your new physician. Your medical history is an important part of effective health care management.
Enclosed you will find a record transfer request with instructions. If you have any questions, please contact DOCUdavit Solutions directly by phone at 1-888-781-9083, fax at 1-866-297-9338 or by email to medicalrecords@docudavit.com. Please note that every individual over the age of nineteen must sign the document. This is required to ensure the privacy of the record.

Tıbbi kayıtlarınızın güvenli bir şekilde muhafaza edilmesi için DOCUdavit Solutions adlı bu konularda uzman bir firmayla düzenleme yapmış bulunuyoruz. İsteğiniz üzerine dosyanızı yeni aile hekiminize ulaştıracaklar. Bu servis için dosyanizin büyüklüğüne göre $30-$95 artı nakliye ve vergi ücreti talep edilecektir.
