πŸ”ˆ REFERRED PAIN - CLINICAL RELEVANCE

Pain from the viscera is poorly localised. As described earlier, it is referred to areas of skin (dermatomes) which are supplied by the same sensory ganglia and spinal cord segments as the nerve fibres innervating the viscera.

Pain is referred according to the embryological origin of the organ; thus pain from foregut structures are referred to the epigastric region, midgut structures are to the umbilical region and hindgut structures to the p***c region of the abdomen.

Foregut – oesophagus, stomach, pancreas, liver, gallbladder and the duodenum (proximal to the entrance of the common bile duct).
Midgut – duodenum (distal to the entrance of the common bile duct) to the junction of the proximal two thirds of the transverse colon with the distal third.
Hindgut – distal one third of the transverse colon to the upper part of the a**l ca**l.
Pain in retroperitoneal organs (e.g. kidney, pancreas) may present as back pain.

Irritation of the diaphragm (e.g. as a result of inflammation of the liver, gallbladder or duodenum) may result in shoulder tip pain.

Referred Pain in Appendicitis

Initially, pain from the appendix (midgut structure) and its visceral peritoneum is referred to the umbilical region. As the appendix becomes increasingly inflamed, it irritates the parietal peritoneum, causing the pain to localise to the right lower quadrant.

πŸ”ˆ DEEP FASCIA OF FOOT

A. The skin and subcutaneous tissue have been removed to demonstrate the deep fascia of the leg and dorsum of the foot.

B. The deep plantar fascia consists of the thick plantar aponeurosis and the thinner medial and lateral plantar fascia. Thinner parts of the plantar fascia have been removed, revealing the plantar digital vessels and nerves.

C. The bones and muscles of the foot are surrounded by the deep dorsal and plantar fascia. A large central and smaller medial and lateral compartments of the sole are created by intermuscular septa that extend deeply from the plantar aponeurosis.

The lymphatic system can be described as the body’s secondary circulatory system, in which lymphatic fluid is moved around the body. It has its own network of vessels, and does a number of important jobs. It absorbs fat from the digestive system, helps to regulate body fluid, and provides the body with its vital natural defences against disease.

Unlike the circulatory system which has the heart to pump it along, the lymphatic system relies on the pulsation of arteries, the pressure difference in the upper body created by breathing, peristaltic movement of the intestine and contractions in the lymph vessels themselves.

The lymphatic system is included in the study of the student reflexologist. They learn how it works and how to work it on the feet. You can see the areas we work for the lymphatic system in red on my chart. πŸ‘£

Over the last 10 years many reflexologist have done further training in a specialized technique called reflexology lymph drainage, or RLD for short.

RLD is an award winning reflexology technique which focuses on stimulating the lymphatic reflexes on the feet. The aim is to cause an effect on the lymphatic system in the body. It is a unique sequence that has been researched and developed by Sally Kay, whilst working in Cancer Care.

I did this training with Sally in 2017 when she travelled β€˜down under’ to teach in both Australia and here in New Zealand.

Since then I have seen many appreciative smiles on the faces of people that I have worked on, as we have shifted and lessened the accumulation of fluid in their body.
