🌸Vaginal Dryness🌸

Vaginal dryness and atrophy can be a B****. I see this commonly in:

1. Postpartum women
2. Breastfeeding women
3. Peri menopausal or menopausal women
4. Women on certain medications like hormone altering therapy post cancer treatment

The most common complaints are:
1. Itchiness
2. Burning
3. Pain with s*x
4. Dryness
5. Inability to wear a tampon due to discomfort

There are many hormonal options out there including premarin and estrace cream or vagifem tablets (applied to the v***a with your fingers) - all of which should be discussed with your medical team to deem if they are appropriate for use in your case.

“BUT I WANT NON HORMONAL OPTIONS” is the response I am generally met with in clinic. In this case, I recommend vaginal moisturizers like:

1. gynatrof
2. hyaluronic acid suppositories
3. coconut oil

🌸Do I have a hernia or diastasis recti?🌸

There is a difference ladies! A diastasis recti is a thinning and stretching of the centre line of tissue called the linea alba. An umbilical hernia would indicate that there is a physical break in that tissue and an organ is coming through.

Generally, these hernias are not bothersome (other than aesthetically) and many women can choose to have them repaired after they are finished having children.

An umbilical hernia is an emergency if the following symptoms are present:
1. Nausea and vomiting
2. Inability to pass a bowel movement
3. Discolouration around the lump (redness)
4. Fever

Any expert can assess for a hernia with simple palpation. Results are often confirmed with an abdominal ultrasound.

🌸Vitamin D and Pregnancy🌸

More and more birth providers are recommending vitamin D supplementation during pregnancy since the start of the pandemic.

Studies investigating the relationship between Covid 19 and vitamin D deficiency are lacking and remain unclear.

What do we know:

1. Vitamin D deficiency in pregnancy can lead to preeclampsia
2. Vitamin D deficiency in pregnancy can lead to rickets (a condition that affects bone development in children)
3. There are many ways to get vitamin D including through sunlight exposure and different food groups

Some of those food groups include: oily fish, liver, and egg yolks.

Constipation in children is usually multifactorial and generally not related to a physiological cause. A lot of the time constipation in children, just like adults, is functional.

Children need the following supports to ensure proper evacuation of stool

✅foot support - and if possible knees above hips
✅ a toilet seat that supports them fully so that they do not feel like they are going to fall in
✅ Handle bars

Did you know that a painful bowel movement is one of the biggest risk factors for functional constipation in children? The Goal is always soft and painless stools.

🌸Endometriosis🌸

Endometriosis is when the uterine lining grows outside the uterus. It can thicken and bleed just as it would inside the uterus when a woman is menstruating. 1/10 women have endometriosis.

The only way to confirm a diagnosis of endometriosis is by laparoscopic surgery and biopsy which is quite invasive.

Symptoms:

Pain with menstruation
Pain with in*******se
Digestive issues
Trouble getting pregnant

There is so much we can do in the pelvic PT realm to manage endometriosis symptoms. Check out this post for lifestyle modifications that can improve symptoms.

Please remember; surgery is not always the answer.

🌸Drink Up Ladies🌸

One of the single most important things you can do for optimal pelvic health is to drink more water. Yes, I’m even talking to you ladies who leak or experience urinary frequency and urgency.

Dehydration contributes to incontinence by making urine more concentrated which irritates the lining of the bladder. It also causes more compact and hard stools which ultimately leads to straining.

Straining and constipation can lead to increased tension in the pelvic floor and all the terrible things we don’t want: prolapse, hemmrhoids, low back pain etc.

If we can avoid additional irritation of the bladder in every day life that’s wonderful - but this is especially important during pregnancy when we already have so much pressure down on the pelvic organs and pelvic floor itself.

Moral of my story: stay hydrated ladies and gents! Your pelvic floor and bladder will thank you.
