Dietitians that work in LTC and with the senior population truly are THE most specialized working RD's...

They are dealing with a patient that has:

- Moderate cognitive decline (Can't fully answer questions)
- 10+ Medical diagnoses, all of which need to be taken into account
- 10+ Medications, 5 of which have nutrient interactions
- Poor food and fluid intake, but can't explain why
- Moderate depression which is affecting their intake

This is just your typical patient working in this field.

Surveyors are looking for the clinical reasoning behind decisions — why an intervention was chosen, how it aligns with the resident’s condition, and how it supports the overall plan of care.

Strong documentation tells the story of your clinical judgment. It shows that your recommendations are intentional, evidence-informed, and appropriate for the resident’s situation.
