Social prescribing enables healthcare providers to refer patients to local, non-clinical community services such older adult programming, exercise groups, transportation services and more, to address social determinants of health like loneliness, isolation, and, in many cases, mental health. It acts as a “link” between medical care and community resources, improving overall wellbeing and reducing pressure on the healthcare system.
