Whiplash PART 2: Role of Sensorimotor Control

A few common and sometimes overlooked symptoms of whiplash are dizziness, decreased balance and or disorientation which might be linked to altered sensorimotor feedback or control.

Your neck muscles and joints are full of sensors that help give your brain information regarding joint position sense and balance (how you sense where your body/head are in space and where gravity is).

A physiotherapist/vestibular therapist can assess and identify areas that might be beneficial to work on and provide exercises geared to improve your overall sense of balance and orientation in space.

Remember, everyone’s experience with pain or whiplash is unique to them. Seek guidance from a healthcare professional to help understand your condition, build a plan (that includes self-management strategies and an active approach), and navigate barriers that may present during your rehabilitation.

Whiplash PART 2: More Than Just a Pain in the Neck 🚗💥

👉 Management
An initial assessment with a primary health care provider is recommended. They will assess your injury/symptoms and find your entry point for rehabilitation and or activity.

Next, symptom management and desensitization. In some patients, this will be important to initiate early. Treatment might include modalities or manual therapy (passive strategies) but it is always good to also have self-management strategies you will be able to carry out on your own at home between sessions.

As symptoms settle, we progress to exercises to restore range of motion, motor control and strength/endurance.

The type of exercise and the frequency these are carried out will depend on the stage of healing you are in, and the ultimate goals of your rehabilitation.

The best exercises will be the one’s you can be most consistent with. This won’t always be the same for everyone! Your therapist will work with you to trial and program what is best for YOU.

Whiplash symptoms can be unpleasant and unsettling, especially when they persist. Your therapist will be there to explain your diagnosis, review expected healing time frames, map out your treatment plan and answer any questions you may have.

Physiotherapists will also communicate and collaborate with your doctor/medical team as needed to optimize your care plan.

Whiplash PART 1: More Than Just a Pain in the Neck 🚗💥

👉 What is Whiplash?
Whiplash is the result of an acceleration-deceleration mechanism that causes the neck to suddenly extend (go backwards) and flex (go forwards).

This mechanism can injure and sensitize various parts of the neck; including
🔴intervertebral joints
🔴discs
🔴ligaments, muscles
🔴nerves
and lead to pain and headaches.

While car accidents are common causes, whiplash can also occur with other mechanisms; like a trip and or fall, or contact sports.

👉 Symptoms of Whiplash
Typical symptoms include
🔴neck pain and stiffness, often appearing right after the injury or even a few hours/days later
🔴 headaches,
🔴shoulder or back pain
🔴dizziness
🔴changes in sensation/numbness
🔴fatigue
🔴sleep disturbances

👉 Classification of Whiplash Injuries

Whiplash injuries are classified based on their associated signs and symptoms:
Grade 1: Complaint of neck pain or stiffness only; no physical signs.
Grade 2: Complaint of neck pain or stiffness with musculoskeletal signs (e.g., reduced range of motion, tenderness).
Grade 3: Complaint of neck pain or stiffness with neurological signs (e.g., weakness, sensory deficits).
Grade 4: Complaint of neck pain or stiffness with a fracture or dislocation.

👉 Recovery
For many, complete recovery is seen within 6-12weeks. For others, whiplash symptoms can linger, with sensitivity and pain lasting for several months.

Factors that seem to influence recovery time include the severity of pain at the time of injury, cervical range of motion, certain psychosocial factors, and external factors like expectations, beliefs and occupational/financial stressors.

🌀 What is BPPV? 🌀

Benign Paroxysmal Positional Vertigo (BPPV) might sound complex, but it is one of the most common peripheral (inner ear) vestibular dysfunctions.

Here's a breakdown of what's happening:

👂 Inner Ear: Inside your inner ear, there's a structure called the utricle (part of the otolith).

It's responsible for housing tiny calcium carbonate crystals which are essentially what allow you to perceive gravity and stay upright.

Usually, these crystals stay put, minding their own business.

🔄 Semi-circular Canals: You also have a set of 3 semicircular canals in each inner ear. These canals are filled with fluid and sensors that allow you to know the direction and speed of your head movements.

💥 BPPV Unleashed: In BPPV, the crystals in the utricle come lose and basically decide to go on an adventure. They slip into one of the semicircular canals, and that's where the trouble starts. These rogue crystals move through the canal when you are in a position gravity can act on them, which triggers sensors and tricks your brain into thinking your head is spinning/moving.

😵 Vertigo Strikes: The result? Vertigo – that feeling like you or the world is spinning/moving when it is not.

BPPV will cause short episodes of vertigo, usually lasting only 10-30 seconds (but sometimes up to a minute). These dizzy spells usually happen when you make specific head movements, like bending forward, getting in/out of bed, looking up, or rolling over in bed.

🚶‍♂️ Managing BPPV: While BPPV itself is not doing damage to your inner ear, it can be distressing and increase the risk of falls. It might even throw off your balance between episodes. The good news? BPPV is highly treatable, and a skilled vestibular therapist can help you get back on steady ground.

Vestibular Migraine is a neurological disorder characterized by recurring episodes of vertigo or dizziness that are usually associated with migraines.

Vertigo is the sensation of spinning or motion when there is no actual movement. In vestibular migraines, this vertigo often presents as a "to-and-fro" sensation.

Vestibular Migraines are not always accompanied by concurrent headaches, which can make the diagnosis a bit tricky; an in-depth history and multi-specialty evaluation are pivotal to make the correct diagnosis.

Key points about vestibular migraines:

🩺Diagnostic Criteria: Vestibular migraines are diagnosed based on specific criteria, including the presence of recurrent episodes of moderate to severe vestibular symptoms (like vertigo) lasting from 5 minutes to 72 hours, a history of migraines with or without aura, and the occurrence of certain migraine features during vestibular episodes.

😵‍💫Complex Diagnosis: Diagnosing vestibular migraines can be complex because the symptoms can mimic other vestibular disorders. A thorough medical evaluation and history-taking are essential for accurate diagnosis.

⚖️Treatment: Treatment for vestibular migraines often involves a combination of strategies. This may include lifestyle modifications, such as identifying and controlling triggers, as well as medications to manage symptoms and reduce the frequency of future episodes.

A vestibular therapist may help you with recalibrating your balance system after an episode and find ways to get active again! (Regular light to moderate physical activity between migraine episodes may also help decrease their frequency and intensity)

Interprofessional Care: Managing vestibular migraines may require collaboration among healthcare professionals, including neurologists, otolaryngologists, and physical therapists. A comprehensive, interprofessional approach can help improve patient outcomes.

🌀 Understanding PPPD 🌀

Have you ever heard of PPPD: Persistent Postural-Perceptual Dizziness?

This week was Balance Awareness Week and we wanted to help share information about often misunderstood vestibular and balance disorders.

Here is a bit of information about PPPD:

🌟 What is PPPD? - PPPD is a chronic vestibular disorder characterized by unsteadiness, dizziness, or non-spinning vertigo that lasts for 90 days or more, often triggered by factors such as movement, busy visual backgrounds, and upright positions. It often causes significant functional limitations to the person affected. It is NOT a diagnosis of exclusion and there is a specific set of criteria that needs to be met for the diagnosis of PPPD.

💡 The Cause: - We are not sure why it occurs but PPPD is often triggered after an initial event: vestibular issue, metabolic condition, medical illness, or psychological distress.

💪 The Power of a Team: - Addressing PPPD isn't a one-size-fits-all journey. A inter professional team including a vestibular therapist, occupational therapist, psychologist, physician, and or specialist collaborate to tailor treatments to your needs.

🤝 How Can a Vestibular Therapist Help? - Vestibular therapists play an important role in helping patients manage PPPD. Treatment might include the use of strategies like cognitive-behavioral therapy and vestibular rehabilitation exercises to address symptoms, build tolerance and improve the quality of life for those affected.

🔬 The Science Behind PPPD: Recent research has shed light on the brain regions involved in PPPD, reinforcing its classification as a functional disorder rather than a structural or psychiatric issue. One way to think of it is PPPD is a “software” or “programming” issue and not a “hardware” issue. Although it is important not to forget that it can co-exist with other vestibular or medical conditions.

📈 Prognosis: A combination of vestibular therapy, cognitive-behavioral therapy and pharmaceutical management have been shown to improve outcomes and quality of life for PPPD patients.
