Last week, we were proud to bring together navigators, healthcare leaders, and community organizations from Elgin, Oxford, and Middlesex-London to continue advancing health system navigation and equity across our region.

Key highlights included:

📞 Health811 updates: expanding digital and live-chat navigation supports, new partnerships with 988 for mental health crisis response, and upcoming initiatives to better assist caregivers and non-clinical inquiries.

👩‍⚕️ HealthCare Connect insights: updates on provincial strategies to connect people to primary care physicians and nurse practitioners and a look at how 235,000 Ontarians are being supported on the waitlist for primary care.

🤝 Cross-OHT collaborations: participants shared projects that embed community navigators and intake coordinators in local health centres to ease the burden on physicians and help connect residents directly to care.

🌍 Health Equity Panel: powerful conversations on Two-Eyed Seeing in Indigenous services, newcomer experiences navigating language and cultural barriers, and the role of mobile and community-based services in reaching people where they are.

We’re grateful to all the speakers, partners, and participants who joined us to share their insights and strengthen our collective work toward a more connected, equitable, and person-centred healthcare system.

Meet Krista Wright, Nurse Practitioner at Tillsonburg District Memorial Hospital’s Rapid Assessment Fast Track (RAFT) Unit.

Krista works with patients of all ages, treating a wide variety of concerns, from injuries and infections to prenatal care and chronic conditions. Her days are full of variety, teamwork, and opportunities to make a difference.

In her time at TDMH, Krista has appreciated opportunities to collaborate with fellow NPs and community services. She values the collaboration that comes with working alongside nurses, doctors, and allied health professionals who share a common goal - to help patients feel cared for and supported.

“This has been such a rewarding role. People within the community have been so welcoming and thankful for the care they have received, and I can say this is the best job I’ve ever had.”

It’s Nurse Practitioner (NP) Week, a time to recognize the vital role NPs play in providing comprehensive healthcare across a variety of care settings in our community.

Nurse Practitioners are highly trained health professionals who provide diagnosis, treatment, and ongoing care for people of all ages. They combine clinical knowledge with a whole-person approach, helping patients understand their health and access the supports they need.

Across Oxford County, NPs are helping improve access to care, reduce wait times, and support people in managing chronic conditions and staying well.

This week, we’re highlighting two incredible local NPs in our community - Krista Wright and Jessica Carrasco-Harding - whose dedication and care make a meaningful difference every day.

We've partnered with local school boards, elected officials, and community agencies to adopt the internationally-recognized Icelandic Prevention Model for youth substance use prevention. It's called Planet Youth!

👉Two new coalitions have formed - Planet Youth Elgin-St. Thomas, and Planet Youth Oxford. Give them a follow Planet Youth Oxford Elgin St. Thomas 🫶

An upcoming survey will help steer local Planet Youth actions:
âś…Students in Grades 9 and 10 in the region are invited to participate
âś…Consent forms have been sent home by school boards to parents/guardians

Oxford HART Hub Update: Central Intake Now Open

Work continues to advance on the Oxford HART Hub – a network of coordinated care designed to support individuals in Oxford County who are facing complex health and social needs, including homelessness, addiction, and mental health challenges.

If you or someone you know is seeking support, you can now self-refer by calling 519-537-2382, Monday–Friday, 8 a.m.–4 p.m. Members of the Oxford HART Hub team will help callers understand their options and connect them to the right care pathway.

If you would like your child to receive either or both of these vaccines, please call our office at 519-539-1111 to schedule an appointment.

Your child may be eligible for the RSV vaccine if:
• Your child was born after April 1st 2025 and who are less than 8 months of age at the time of immunization
• They have not previously received an RSV vaccination

Last week, OHT team members Emily and Diane, spoke as part of the Learning Unlimited Program at the South Gate Centre in Woodstock.

Diane, OHT Clinical Coach, Palliative Care, presented information on the palliative approach to care and advance care planning. Emily, Project Coordinator- Community Engagement, shared our new Aging Well resource for community members, as well as other OHT initiatives like the Tillsonburg Unattached Care Clinic, HART Hub and Connect My Health.

It was great to connect with over 250 community members and have meaningful conversations about aging in our health system. Attendees asked thoughtful questions and took home over 200 materials. Thank you for having us!

We’re saying a heartfelt thank you to Heather, one of our long-time Community Council members, for her years of dedication and advocacy.

Heather has been involved with the Oxford OHT since the very beginning, bringing her valuable perspective as a caregiver and helping shape how care is delivered across our community.

As she steps away from the Council, Heather shared this message to encourage others:

“I would never have expected by joining this group at the beginning how much I would have gained. I’m so grateful for the opportunity... You need to be an advocate and share your story - that’s how we learn from one another.”

Thank you, Heather, for everything you’ve contributed to the OHT and to Oxford County’s health system. 💙
