The American Heart Association and American College of Cardiology have recently released updated hypertension guidelines. Importantly, these new guidelines represent the first time that specific recommendations have been proposed for (PA).

While previous guidelines have noted that all causes of secondary hypertension should be explored before diagnosing a patient with resistant hypertension, including PA, they failed to do more. The updated guidelines now describe in more detail patients that should be specifically screened for PA and, importantly, recommend not washing patients out of most antihypertensive agents before diagnostic testing. This is an important element since PA is diagnosed at very low rates, in part due to cumbersome diagnosis. The new guidelines should raise awareness of PA among cardiologists and support an increase in diagnosis among patients with hypertension, particularly those early in their disease.

The Endocrine Society recently published new clinical practice guidelines for (PA). The revised guidelines are intended to improve the identification and diagnosis of patients with PA, a disease with significant long-term consequences but very low diagnostic rate.

Key among the new recommendations is that aldosterone suppression testing is now only indicated if the screening results show an intermediate probability for lateralizing PA. Additionally, the guidelines allow the empiric use of medical therapy in the absence of testing the blood for aldosterone and renin levels (the ‘aldosterone-to-renin ratio’) if the feasibility for such testing is low. The authors note that even subclinical (i.e. early) PA is negatively associated with cardiovascular disease indicators and that ‘the morbidity and mortality associated with PA are largely preventable.’

Too much sodium is associated with high blood pressure and can lead to stroke. A recently published study by Ding et al. enrolled patients with a history of stroke to evaluate the use of salt substitutes on the incidence of recurrent stroke.

In a substudy of the Salt Substitute and Stroke Study (SSaSS) in China, more than 15,000 stroke patients were given either regular salt or a salt substitute (75% NaCl, 25% KCl) and followed for an average of five years. The primary outcome was diagnosis of recurrent stroke following adjudication by an independent committee.

Investigators found that using the salt substitute reduced the risk of both fatal and nonfatal recurrent stroke, providing further evidence that sodium-reducing measures may reduce some secondary outcomes of high blood pressure. However, caution should be taken in patients who may be at-risk for hyperkalemia.

As has been reported in other studies, data from the Japan (PA) Study confirm that high aldosterone levels in patients with PA increase the risk for cardiovascular (CV) events compared with patients with essential hypertension.

In their study, Ohno et al. also investigated what additional factors may confer a higher CV risk to PA patients. The analysis showed that PA patients with hypokalemia (low potassium) or the unilateral subtype (a benign adenoma on one adrenal gland) have an even higher risk of cardiovascular disease. Further, they provide evidence that aldosterone levels higher than 125 pg/mL also confer a higher risk.

The authors recommend that patients with high aldosterone, hypokalemia, or unilateral disease be preferentially diagnosed for PA and treated with PA-specific drugs.

A recent review by Jones et al. in JAMA Cardiology highlights two studies that may provide the “tipping point” in the recognition that voluntary sodium reduction may not be sufficient to address the increasing incidence of high blood pressure and consequent cardiovascular disease in advanced societies.

The first study was done in South Africa to evaluate the success of a government initiative to require food manufacturers to reduce sodium content in some prepared foods. This may be particularly important in areas where most sodium intake is derived from processed foods.

The second study was a pre-specified analysis of the Salt Substitute and Stroke Study (SSaSS) which demonstrated that by adding a potassium-rich salt substitute to the regular diet, patients with a history of stroke were able to reduce the risk of a second stroke or all-cause mortality. This approach may be more beneficial in areas where sodium intake is mostly through foods prepared at home although the SSaSS authors do note that patients with ongoing disease such as kidney disease or diabetes or patients on potassium-sparing drugs should consult a physician before changing their diet.

Ultimately both studies demonstrate the importance of reducing dietary sodium and will hopefully encourage more public health advocacy.

While (PA) may not be a rare disease per se, it's rarely diagnosed in patients due to a lack of awareness of the long-term CV risks. This , consider speaking with your physician about being screened for PA — especially if you have high blood pressure and find yourself at risk for obesity-related complications.
