Today we’ll take a look at a small movement – a micro-exercise that is appreciated not only by in their clinical practice, but also by almost anyone with knee-related issues, especially chronic ones. Naturally, it is also highly relevant for elite athletes in the context of knee joint health.
🦵🏻 When we talk about the joint, in 99% of cases we think of flexion and extension. However, there is another important movement that occurs when the knee is unlocked, meaning during flexion—internal and external rotation of the tibia relative to the femur. This movement is essential for knee health. If we further add active internal rotation with resistance, it can become an excellent exercise that can be performed long term, while simultaneously activating the medial gastrocnemius and strengthening both the adductor longus and the medial hamstrings.
👨🏽🔬 Scientific research emphasizes that proper rotation of the femorotibial joint is not merely an accessory movement, but a key component of healthy knee biomechanics. Its dysfunction represents a significant biomechanical issue with direct clinical consequences; restricted rotation is commonly observed, for example, in osteoarthritic knees.
Therefore, do not neglect this movement—especially if you observe increased tibial rotation relative to the femur (which may be a risk factor, for example, in ACL-related cases), but also as part of any knee rehabilitation or knee pain prevention program.
