A 55 years old male patient was complaining of inability to smile or eat and asked us to make him an implant supported prosthesis.

Upon taking the dental and medical history and checking the x rays, a large cyst had found in the upper jaw which had a history of recurrence since 1994.

The treatment plan was to take an excisonal biopsy. Which showed no tumer cells and only odontogenic cyst.

A very good curettage was done to remove the cyst with safety margins.

- GBR in the cyst cite
- A full arch implant supported restoration was planned.
6 implants were placed and after 6 months a PFM restoration was constructed.

٢- زراعة أسنان في الفك العلوي منطقة الضروس مع رفع للحاحز الأنفي
٣- جراحة لثة تجميلية للفك العلوي المنطقة الأمامية مع تركيبات الزيركون التجميلية للأسنان الأمامية.
A Full mouth Rehabilitation Case:
Treatment steps:
1- implants supported PFM restorations for the lower jaw.
2- implant placement in the upper posterior region with sinus lifting.
3- gengivectomy & osteoplasty for the anterior region of the upper jaw with zirconia crowns over the 6 anterior teeth

a 60 years old female patient was complaining of inability to eat due to multiple teeth extractions and severely atrophied Jaws .
She strictly Refused the Complete denture treatment option.
Then, a detailed explanation of the treatment plan was presented to her which was as the following:
1- Reconstruction of the maxilla with bone
2- 6 implants with PFM prosthesis for the lower jaw with minor bone grafts at the open sockets after extraction.
4 implants for the maxilla with full arch PFM prosthesis.
Treatment time would have been 1.5: 2 years
She accepted the treatment plan.
Treatment steps:
1- Surgery for the Mandible
6 implants were placed with the aid of a vacuum formed stent to locate the crowns sites.
Osseodensification concept was used for the thin ridge of the mandible by using Denssah Burs
2- Surgery for the maxilla
by using Allografting bone blocks ( corticocancellous & cancellous ) types with split mouth technique which is using the corticocancellous blocks in the right side and the cancellous blocks in the left side to test the bone quality and quantity later on .
maxilla decortication was made with ( piesotome device ).
time left for implants osseointgration in the mandible was 4 Months
and time left for boneblocks healing was 9 months
during the healing period a complete removable denture was constructed as temporary restoration
3- Prosthesis for the mandible
by using the upper RPD as an opposing restoration A full arch PFM restoration was constructed.
4- 2nd surgery to the maxilla
A fully guided implant placement surgery was done to not interrupt the bone healing and to avoid bone resorption and cutting the blood supply after new flap elevation.
All on 4 Concept was the decision of treatment:
2 straight implants in the anterior region and 2 tilted implants in the premolar regions placed very carefully between the fixation screws which used for bone blocks fixation.
5- prosthesis for Maxilla
after 6 months for implants osseointegration
ful arch PFM was constructed
Occlusion was adjusted and the patient was asked to come for any more adjustments needed.
the Patient is happy and also the Implantologist
