Could you please tell me what the solution is?”

“If the double chin is mild and the skin is tight, with superficial fat above the platysma, then simple liposuction can solve the problem.
If there is skin laxity and muscle looseness, we resort to a neck lift, which is the classic neck tightening procedure.
Sometimes there is also enlargement of the salivary glands and the deeper muscles, and in those cases we need a deep plane neck lift.”

Extreme transformations and overdone trends may look glamorous online — but they aren’t always safe or realistic.
True aesthetic surgery is about balance, subtle correction, and natural rejuvenation. It should never try to change who you are, only enhance your natural beauty.
Some invasive procedures carry extremely high risks and require careful patient selection. Factors like smoking, skin quality, medical history, and healing capacity all matter.
During consultation, our priority is education, safety, and long-term results. Ethical practice sometimes means saying no when a request isn’t safe or medically justified.

First of all, we choose the implant based on specific measurements that suit your body.
What you’re describing is a repeated scenario.
Before surgery, the patient becomes anxious and asks for a larger size, worried that the result might not be enough.
After surgery, the patient is extremely happy.
Then, after six months, she comes back asking for a bigger size.
We can always go bigger, but only in a way that fits the measurements and the nature of your body.

Professor Dr. Ashraf Abolfotooh was recently invited to speak at a meeting of the Neurology Department, Faculty of Medicine, Cairo University (Kasr Al-Aini), focused on patients suffering from chronic headaches and migraines.

As a plastic and reconstructive surgeon, Professor Dr. Ashraf Abolfotooh introduced neurologists to the role that nerve compression and carefully selected surgical techniques may play in relieving chronic pain in a subset of patients who do not respond to conventional therapies.

While surgical intervention in this context is not yet widely considered locally, this approach is already established in several parts of the world. The discussion opened the door to new academic dialogue and the possibility of expanding treatment options for patients in Egypt.

As a next step, efforts are underway at Cairo University to begin identifying appropriate candidates and developing a structured, evidence-based protocol, with the aim of making this form of treatment safely and responsibly available locally.

Throughout his career, Professor Dr. Ashraf Abolfotooh’s work has focused on restoring function and improving quality of life through advanced reconstructive techniques, including microsurgical and nanosurgical procedures used in post-cancer breast reconstruction, lymphedema surgery, trauma reconstruction, and selected chronic pain conditions. His commitment has consistently been to bring proven, internationally established reconstructive methods to Egypt, reducing the need for patients to seek treatment abroad.

Initiatives such as this reflect a continued dedication to responsible innovation in reconstructive surgery and to expanding patient care through education, interdisciplinary collaboration, and academic development.







تمت دعوة الأستاذ الدكتور أشرف أبوالفتوح مؤخرًا لإلقاء محاضرة في اجتماع قسم المخ والأعصاب بكلية طب قصر العيني – جامعة القاهرة، والذي ركّز على المرضى الذين يعانون من الصداع المزمن والصداع النصفي.
وبصفته أستاذًا واستشاريًا في جراحة التجميل والترميم، قدّم الأستاذ الدكتور أشرف أبوالفتوح عرضًا علميًا حول دور انضغاط الأعصاب وبعض التقنيات الجراحية المختارة بعناية في تخفيف الألم المزمن لدى فئة من المرضى الذين لا يستجيبون للعلاجات التقليدية.
ورغم أن التدخل الجراحي في هذا السياق لا يُعد خيارًا شائعًا محليًا حتى الآن، فإن هذا النهج العلاجي مطبّق ومعتمد في عدة دول حول العالم. وقد فتح النقاش المجال أمام حوار أكاديمي جديد وإمكانية توسيع الخيارات العلاجية المتاحة للمرضى في مصر.
وكخطوة تالية، تُبذل حاليًا جهود داخل جامعة القاهرة للبدء في اختيار الحالات المناسبة ووضع بروتوكول علمي منظم قائم على الأدلة، بهدف إتاحة هذا النوع من العلاج بشكل آمن ومسؤول على المستوى المحلي.
وطوال مسيرته المهنية، ركّز عمل الأستاذ الدكتور أشرف أبوالفتوح على استعادة الوظيفة وتحسين جودة الحياة من خلال تقنيات ترميمية متقدمة، بما في ذلك الجراحات الميكروسكوبية والنانو جراحية المستخدمة في ترميم الثدي بعد الأورام، وجراحات الوذمة اللمفية، وجراحات الإصابات، وبعض حالات الألم المزمن المختارة. وقد كان التزامه الدائم هو نقل الأساليب الترميمية المتقدمة والمثبتة علميًا إلى مصر، بما يقلل الحاجة إلى سفر المرضى للعلاج خارج البلاد.
