Abstract. Introduction: Hypoxic-ischemic encephalopathy (HIE) is a leading cause of neonatal mortality and neurodevelopmental disabilities. Therapeutic hypothermia (TH) is the standard of care, but optimized sedoanalgesic strategies remain critical. Dexmedetomidine shows promise as an alternative to...

To evaluate the safety and efficacy of dexmedetomidine as the primary sedative in neonates. Single center, retrospective study of mechanically ventilated neonates sedated with dexmedetomidine (study group) or opioids/benzodiazepines (control group). Primary outcome was requirement of opioids/benzodi...

Contraindications to iron supplementation
☠️☠️

👀Cases of hemolysis, such as Rh or ABO incompatibility, disseminated intravascular coagulation, thalassemias, G6PD or sickle cell anemia where the red blood cell components remain in the body, transfused red cells add to the total body iron load and therefore supplemental iron is not indicated.
💣💣Note:For the most part in the NICU, red blood cell transfusions are given to counteract the effects of phlebotomy losses. In these scenarios, ongoing iron supplementation is safe and should be
continued if indicated by iron status parameters. This is because the iron
housed in red blood cells has been lost from the body as phlebotomized blood.

👀Active infection with gram negative
bacteria as it is siderophilic.

👀Iron overload (ferritin values above 350–400 ng/mL)

👀Hemochromatosis, gestational alloimmune liver disease (GALD) and certain inborn errors of metabolism

Infants randomised to a higher platelet transfusion threshold of 50×109/L compared with 25×109/L had a higher rate of death or significant neurodevelopmental impairment at a corrected age of 2 years. This further supports evidence of harm caused by high prophylactic platelet transfusion thresholds in preterm infants.

Extended caffeine reduced the burden of IH in very preterm infants and may reduce inflammation. Further study is needed to determine if this effect of caffeine is associated with reduced risk of adverse outcomes.
