خشونة مفصل الكتف
يعتبر مرض خشونة مفصل الكتف اقل شيوعا من مثيله فى المفاصل الاخرى مثل الركبة, و لكنه يؤثر بصورة كبيرة على حركة الطرف العلوى كله وذلك لعدم قدرة المريض على توظيف الطرف العلوى بصورة فعالة. وتحدث هذة الخشونة اما نتيجة لاصابات قديمة (مثل الكسور) او نتيجة لاصابات متكررة مهملة مثل الخلع المتكرر لمفصل الكتف. وقد تحدث هذة الخشونة بصورة اولية (بدون سبب ظاهر) وذلك نتيجة لعوامل وراثية او بيئية.
يعانى مريض خشونة مفصل الكتف من الآم شديدة مع حركة المفصل مع محدودية تلك الحركة تبعا لدرجة تقدم المرض. يحتاج هذا المريض لعمل الاشعات العادية و احيانا الاشعة بالرنين المغناطيسى و ذلك للوقوف على درجة تأثير تلك الخشونة على عظام المفصل و تحديدا حق عظمة الكتف .

مراحل العلاج:
١. العلاج التحفظى: يعتبر العلاج التحفظى ناجحا فى حالات الخشونة البسيطة فى مفصل الكتف. ويتكون من: ادوية مضادة للالتهابات, جلسات العلاج الطبيعى و تمرينات اعادة الحركة.

٢. منظار الكتف الجراحى: ويقوم هذا العلاج على استعمال منظار الكتف فى ازالة الاجزاء المتآكلة من غضروف الكتف. ويكون هذا العلاج ناجحا فى الحالات المتوسطة و التى لا تشمل اصابة حق مفصل الكتف بدرجة كبيرة.

٣. تغيير مفصل الكتف: و تستعمل هذة الطريقة فى الحالات المتقدمة من تآكل المفصل و التى لا يتوقع نجاح اى من الطرق الاخرى. و فى هذه الجراحة يتم أستبدال الأسطح المتآكلة من عظام مفصل الكتف بأجزاء صناعية بحيث يختفي الألم الناتج عن إحتكاك العظام ببعضها, و يزداد مدى الحركة فى الكتف.

تقدم وحدة جراحات الطرف العلوى و اليد( قسم جراحة العظام -جامعة المنصورة) الخدمات العلاجية السابقة سواء مجاني أو تأمين صحى أو علاج على نفقة الدولة بنسبة نجاح عاليه خاصة تركيب مفصل الكتف الكامل أو المعكوس أو مفصل التغليفى (كوبلان) . هذه بعض حالات الوحدة.

Mansoura International shoulder course (Basic condensed and practical lectures, Cadaveric Workshop, Live Surgery and Models Workshop)
3-5.11.2016
Organized by the Shoulder Group of the Upper Limb Unit, Mansoura University and in collaboration of a large Group on international renominated Shoulder Surgeons.
Till now we have Prof Imhoff (Germany), Prof Jerosch( GErmany), Prof Giovanni (Italy) together with a large group of international and national shoulder experts.
Main Idea and frame of the course is to:
Meet the inventors of the techniques
Ask the experts and learn from them directly.
learn the practical tips and tricks
get aware of debates and controversies in arthroscopic and shoulder Surgery.

تم بعون الله تعالى وصول مايكروسكوب الجراحات الدقيقه لوحده الطرف العلوي، و قد تم الحمد لله اجراء اول عمليه للضفيره العصبيه اليوم brachial plexus injury treated by motor neurotization ، و هنا نود ان نشكر استاذنا و معلمنا الاستاذ الدكتور يحيى بسيوني Yehia Basyoni لدعمه المتواصل و جهده الحثيث لإيصال كل جديد لوحده الطرف العوي و اقسام جراحه العظام.
