ይህ በምስሉ ላይ የሚታየው ታካሚያችን ለረዠም ጊዜ በወሰደው የ "Dexametason and Diclofenac" ቅልቅል መርፌ ምክንያት ባጋጠመው የመጨረሻ ደረጃ ላይ በደረሰ የሁለቱም ዳሌ ራሶች መበስበስ ምክንያት በከፍተኛ የሆነ ህመም፣ሰቃይ እንዲሁም ለመራመድ በመቸገር ለረዠም ጊዜ ሲሰቃይ ነበር።

ወደ እኛ በመምጣትም የሁለቱም ዳሌዎች መገጣጠሚያ ቅየራ ተሰርቶለታል።

ይህ ቅየራ በዳሌ ፊት ለፊት በኩል ጡንቻ ሳይቆረጥ በተመሳሳይ ሰአት የሁለቱንም የዳሌ መገጣጠሚያዎች ቅየራ የተሰራለት ሲሆን ከተሰራ ከሁለት ሰአታት በኃላ በቀላሉ መራመድ ችሏል።

በ2 በሳምንታት ውስጥም ያለምንም ድጋፍ በምስሉ ላይ እንደሚታየው ሽር ብትን ማለት ችሏል።

This patient had been suffering for a long time from severe pain, disability, and difficulty walking due to avascular necrosis of both femoral heads.

This condition was caused by prolonged use of combined injections of Dexamethasone and Diclofenac.

After presenting to our center, the patient underwent Bilateral Direct Anterior Approach Total hip joint replacement.

The surgery was performed using the Direct Anterior Approach, a muscle-sparing technique, allowing both hip joints to be replaced in the same surgical session without cutting muscles.

The patient was able to start walking just two hours after surgery.

Following surgery, our patient experienced a relief from the severe pain and suffering.

Within just two weeks, the patient was able to walk independently without any support.

ከጉልበት መገጣጠሚያ ቅየራ ቀዶ ህክምና በኋላ ታካሚዎቻችንን ወደ ቤት ከመሸኘታችን በፊት በዋናነት የምናያቸው 3 ወሳኝ ነገሮች፡-

1️⃣ ያለሰው እርዳታ ወደ መፀዳጃ ቤት መሄድ መቻል። ታካሚው በራሱ ወደ ሽንት ቤት የመሄድ እና የመጠቀም አቅም ሊኖረው ይገባል።

2️⃣ የጉልበት መታጠፍ መጠን፡- ታካሚው በምቾት ለመቀመጥ እና የእለት ተእለት እንቅስቃሴዎችን ለማድረግ እንዲያስችለው፤ የተሰራው ጉልበት ቢያንስ 90 ዲግሪ መታጠፍ አለበት።

3️⃣ በራሱ መራመድ መቻል፡- በድጋፍ ወይም ያለድጋፍ፤ ደረጃ መውጣትና መውረድን ጨምሮ በራሳቸው መንቀሳቀስ መቻል አለባቸው።

3 things we look for before discharging a patient home after Total Knee Arthroplasty (TKA):

1️⃣ The patient should be able to go to the toilet independently.

2️⃣ Knee flexion should reach at least 90 degrees, allowing comfortable sitting and daily activities.

3️⃣ Independent ambulation, including climbing stairs, with or without assistive devices.

Early Rehabilitation After Surgery: Core component of Enhanced Recovery After Surgery (ERAS)

Early postoperative mobilization is central to ERAS. By minimizing prolonged bed rest, optimizing pain management, and ensuring coordinated care among orthopedic surgery, anesthesia, nursing, and physiotherapy teams, we can significantly reduce risks such as functional decline due to inactivity, thromboembolism, and delayed recovery.

In this case, our 79-year-old patient, with hypertension, diabetes mellitus, and chronic kidney disease, sustained a left femoral neck fracture.

We performed a cemented modular bipolar hemiarthroplasty, as it is safer,faster and providing stable fixation while minimizing surgical and anesthetic stress.

For medically complex patients like this, early mobilization is very important with effective pain control.

ከቀዶ ሕክምና በኃላ በተቻለ በሰዓታት ዉስጥ እንቅስቃሴን መጀመር ከቀዶ ህክምና በኋላ ፈጥኖ እና በተሻሻለ ሁኔታ ማገገም (ERAS ) መርሆች ውስጥ አንደኛው እና ዋነኛው ነው:: ረጅም ጊዜ አልጋ ላይ ተኝቶ መቆየትን በመቀነስ፣ ህመምን በአግባቡ እና በበቂ ሁኔታ በመቆጣጠር እና በአጥንት ቀዶ ሕክምና ፣ በሰመመን መድሃኒት ባለሙያዎች ፣ ነርሶች እና ፊዚዮቴራፒስት መካከል የተቀናጀ ትብብር በማረጋገጥ፣ በእንቅስቃሴ አለመኖር ምክንያት የሚመጣን የደም መርጋት ፣ የሳንባ ኢንፌሽን ፣ የማገገሚያ ጊዜ መራዘም እና በበቂ ሁኔታ አለማገገምን የመሳሰሉ አደጋዎችን እጅግ እንቀንሳለን።

በዚህ ምሳሌ፣የ 79 ዓመት ታካሚያችን የደም ግፊት ፣ የስኳር በሽታ እና የገፋ የኩላሊት ሕመም አላቸው። የዳሌ አንገት አጥንት ስብራት አጋጥሟቸው ነበር::

የአጥንት ስሚንቶን ተጠቅመን ሁለት ተንቀሳቃሽ ራስ ባሉት የግማሽ የዳሌ መገጣጠሚያ ቅየራ ሰራንላቸዉ:: (cemented modular bipolar hemiarthroplasty)

ይህም የተመረጠበት ምክንያት ተደራራቢ የጤና እክል ስላሏቸው እና ከሕመም ተጋላጭነታቸው ፤ ከ አጥንት መሳሳት እና የመዳን እሀክል አንፃር ፈጣን ፣ ትክክለኛ እና ጠንካራ የሆነው አማርጭ ስለሆነ ነው ::

ከሰመመን መዳኒት ጋር ተያይዞ ሚመጣውንም ጉዳት ይቀንሳል::
ለእንደዚህ ዓይነት የሕመም ጫና ላለባቸው ታካሚዎች፣ በተቻለ ፍጥነት እንቅስቃሴ ማስጀመር እና በተገቢ ሁኔታ ህመምን ማስታገስ አስፈላጊ ነው።

ከቀዶ ህክምና በኋላ የሚደረግ ዘገምተኛና ባህላዊ የማገገም ሂደት በሆስፒታል የመቆያ ጊዜን ያራዝማል፣ የችግር መከሰት ዕድልን (እንደ ኢንፌክሽን ወይም የመንቀሳቀስ መቀነስ) ይጨምራል፣ እንዲሁም ወደ ዕለታዊ እንቅስቃሴ ለመመለስ አላስፈላጊ መዘግየቶችን ያስከትላል። ይህ አካሄድ በአካልም በገንዘብም ውድ ነው።
