Are you passionate about making a difference in people’s lives? Do you have experience in the care sector and love supporting a team?

We are currently looking for two dedicated and organised Care Coordinators to join our friendly team. As a Care Coordinator, you’ll play a key role in ensuring the smooth running of our care at home services, supporting both clients and carers alike.

📝 What You’ll Be Doing:

• Scheduling and coordinating care visits and rotas
• Carrying an On Call Phone on a rota basis in evenings/weekends
• Supporting carers with day-to-day operations
• Ensuring high standards of care delivery
• Communicating with other health professionals
• Attending meetings with carers, client and health care professionals
• Building strong relationships with clients and families

✅ What We’re Looking For:

• Experience in Health & Social Care
• Excellent communication & organisational skills
• A caring and compassionate nature
• A can-do attitude and team spirit
• Ability to work well under pressure
• Good problem solving skills
• SVQ in Health & Social Care (Preferred but not essential as there are opportunities to complete this within your role)
