💬 “What support is available after a hospital discharge to prevent another fall or readmission?”

Returning home after a hospital stay can feel reassuring — but it can also be a vulnerable time. Strength, mobility and confidence may not yet be fully restored, and without the right support, the risk of falls, complications, or hospital readmission can increase.

Planning the right care after discharge can make a real difference in supporting recovery and helping people stay safe at home.

At Ashridge, we offer a range of home care options tailored to each individual situation. Depending on needs, this may include hourly visiting care or 24/7 live-in care, helping people recover safely in the comfort and familiarity of their own home.

For those requiring more specialist support, care can also include:
✅Rehabilitation care following illness, injury or surgery
✅Specialist care for conditions such as dementia, Parkinson’s or cancer
✅Complex care for people living with chronic or progressive conditions, including support with clinical needs such as catheters or stomas

Good recovery also depends on rebuilding everyday routines and confidence. Support with daily living skills — such as getting dressed, preparing meals, or even walking to the local shop — can play an important role in helping people regain independence and stability after a hospital stay.

Someone supported by a live-in carer is also more likely to enjoy the food and drink they want, helping them stay properly nourished and hydrated. This supports overall health and quality of life — and can reduce the risk of urinary tract infections, which are a common contributing factor to falls in older people.

Research also shows that people receiving live-in care experience around a third fewer falls and about a quarter fewer hip fractures compared with those in residential care settings.

Hearing loss affects 18 million people in the UK — 1 in 3 of us, most commonly through age-related changes that develop gradually over time.

Without the right support, hearing impairment can lead to isolation, frustration, reduced confidence and increased risks such as depression and dementia.
