Ultrasound-guided foam sclerotherapy of the saphenous trunks is associated with a low 5-year recurrence rate and improved quality of life in patients with chronic venous disease: A multicenter study. In press – Journal of Vascular Surgery
A large multicentre study finds that Ultrasound Guided Foam Sclerotherapy is effective in managing reflux in the truncal veins with low recurrence rates and high patient satisfaction.

Article Highlights
Type of Research: Multicenter retrospective cohort study
Key Findings: Ultrasound-guided foam sclerotherapy in 346 patients with primary chronic venous disease resulted in a 5-year disease-free rate of 77.7 ± 2.16%, with most recurrences in patients with a saphenous trunk diameter of >8 mm (91.3% vs 46%). In post-treatment surveys, patients reported improved quality of life (P < .001).
Take Home Message: Ultrasound-guided foam sclerotherapy is an effective, long-term, safe, and well-accepted procedure for saphenous trunks incompetence.

The study attempts to test whether ultrasound-guided foam sclerotherapy (UGFS) is a durable treatment for incompetent great saphenous vein (GSV) and incompetent small saphenous vein (SSV) in primary chronic venous disease (CVD), Clinical-Etiology-Anatomy-Pathophysiology (CEAP) clinical grade 2 to 4....

💉 Unwanted Staining After Microsclerotherapy: A Case Study and Discussion 💉
We've all seen the amazing results Microsclerotherapy can achieve for thread veins, but what about those occasional cases with unexpected staining?
This 66-year-old patient experienced significant brown staining after her second treatment session, despite initial improvement and the use of a lower concentration of STS (0.1%).
Case Details:
● Patient: 66-year-old female with telangiectasia at the medial malleolus.
● Assessment: No proximal reflux or varicose veins detected on colour duplex.
● Treatment 1 (May 2024): 10 ml 0.1% STS with class II compression for three days.
● Results: Initial improvement, patient delighted with cosmetic outcome.
● Treatment 2 (September 2024): Significant brown staining observed, particularly in proximal thread veins.
🤔 Clinical Question:
This case raises important questions about our practice:
● What sclerosant agents and concentrations do you prefer for Microsclerotherapy?
● How often do you encounter staining in your patients?
● What strategies do you employ to minimise the risk of staining?
● How do you manage patients who develop significant staining after treatment?
💬 Let's Share Our Knowledge! 💬
We encourage all healthcare practitioners with experience in Microsclerotherapy to share their insights and best practices in the comments. Your expertise can help us all improve patient outcomes and minimise complications.
