⭐ Quality Improvement Project boosts self-management and enhances support for stroke patients and their carers ⭐
A huge congratulations to Dr Aimee Hayter, Helena Reason and the therapy team on Runfold Ward, Farnham Hospital on the successful completion of this impactful Quality Improvement Project! 👏
This project focused on delivering a one-hour, personalised psychoeducation session for stroke survivors and their carers prior to discharge, aiming to improve self-management, confidence, and preparedness for life at home. The intervention was grounded in evidence, co-designed for inclusivity (including those with cognitive impairment and aphasia), delivered by both psychologists and wider MDT, and evaluated using both quantitative outcomes and thematic analysis.
Key results from the study sample included:
✅ Significant improvements in patient activation measures both patients’ and carers’ confidence and readiness to self-manage sustained one month post-discharge
Early modelling indicating a 186% ROI - £1.86 saved in future healthcare use for every £1 invested
✅ High satisfaction - one-month post-discharge, 88% of patients found the session useful and 100% found the accompanying booklet informative
✅ Positive qualitative feedback highlighted the value of personalised support, reassurance from knowledgeable staff, and increased awareness of community services
✅ Ongoing need for embedded psychologists in stroke rehab to support the wider MDT to deliver this intervention
