Yesterday our Social Prescribing team ran another drop-in at Ashton Medical Group & welcomed an amazing 20 patients to the session! The patients looked through the Ashton Activity Sheets to find out about groups, activities & services that they can access in their local area. Our Social Prescribers will then follow up with these patients in the next few weeks to give them a call to find out if they need any further signposting.
A few of the patients connected with Rachel from WorkWell from Work & Skills Tameside to begin to take some steps towards finding employment.
What is the criteria for me to access Social Prescribing in Ashton?
* Over 18 years old
* Registered with an Ashton-based GP surgery
* Non-urgent, non-medical need
* Not in crisis with your mental health
* Ready to engage with signposting
