We here at Chilton House are working to keep our residents safe and well. Due to the Coronavirus, our house is closed to all visitors and it is likely to stay that way for the foreseeable future.
We know that this is a tight knit community and we like to help one another so during these tough times if you have been asked not to come into your place of work because it has had to close, maybe you would like to apply to work in our hospitality, housekeeping or care teams?

We have three kind and wonderful physiotherapists at Chilton House (this one is Jenny) and almost all residents will meet one of them the day after they arrive here for a chat and an initial physio assessment. ⠀

We pride ourselves on the highly tailored 1-1 therapy we can offer every resident, whether they’re staying for post-operative R&R (we see a lot of hip and knee replacements) or for the longer term. ⠀⠀⠀⠀⠀⠀

DEPARTMENT PROFILE:

To provide personal care and promote well- being and independence for all residents within a safe and caring environment.

JOB PROFILE:

To lead and develop the care team and ensure delivery of exemplary patient centered care which promotes independence and well-being.

KEY RESPONSIBILITIES:

Provide professional leadership and promote best practice in all areas of care delivery
Be responsible for residents’ personal care and well being, ensuring that each resident receives person centered care.
Be responsible for each residents care plans ensuring they are current and reflect each residents needs.
Work as a multidisciplinary team to deliver the care
Safely administer and accurately record medicines for residential residents
Support with medication renewals and medication audits
Manage the accident and incidents reported, follow up, and provide monthly analysis.
Allocate tasks and ensure they are completed
Maintain records in accordance with all policies, procedures and legislation
Manage the care rota in line with dependency and ensure correct staffing levels at all times.
Manage annual leave requests within the team
Manage recruitment and retention, managing performance, appraisals, supervisions, training, meetings, audits
Induct new members of the care team
Communicate in an effective, timely and courteous manner with everyone
Ensure that confidentiality is maintained at all times
Follow all company policies and procedures
Take responsibility for attending all required courses and undertake self -directed learning
Assist other Chilton House teams as and when required
Perform any additional duties as required by the Managing Director

o Ensure that person centered care plans are being delivered
o Liaise with local GP practice and wider external multi- disciplinary team
o Take responsibility for ordering, storing, administering and disposing medication in accordance with legislation
o Communicate in an effective, timely and courteous manner with everyone
o Ensure that confidentiality is maintained at all times
o Follow all company policies and procedures
o Take responsibility for attending all required courses and undertake self -directed learning
o Assist other Chilton House teams as and when required
o Perform any additional duties as required by the General Manager
