You are invited to attend our drop in session. This session can be helpful to anyone in the community. You will have the chance to meet our social prescribing team and discuss services in you area.

Many things that affect our health can’t be treated by doctors or medicine alone. Like loneliness, debt, or stress due to financial pressures or poor housing. Social prescribing connects people to non-medical support to address these issues and other unmet needs.

If this is something that could be helpful to you, please pop into: Ibbison Court Community Centre, Central Drive, FY1 4AT on Friday 21st November 2025 between 9:30am and 12:45pm πŸ™ŒπŸ€

πŸ“’ Important Update: We Are Moving to Total Triage – From 01st October 2025
As part of national NHS contract changes introduced by the government, we are moving to a Total Triage system.

πŸ”Ž What is Total Triage?

Total Triage means that all requests for appointments – whether made online, by phone, or in person – will first be reviewed by a clinician before an appointment is booked.

This helps us ensure that every patient gets the right care at the right time, based on clinical need.

πŸ•’ How it works:

You request help – via our online Patchs form or by calling the practice.
An experienced clinician reviews your request –
We decide the best next step, which may be:
β€’ An appointment with a GP – the clinician may arrange some investigations like a blood test, X-ray or scan before the appointment so that the results are ready to discuss in time. Face-to-face care always remains available, both routine and urgent (same day) appointments when clinically appropriate.
β€’ A consultation with another member of our clinical team β€” such as a paramedic, physiotherapist, pharmacist, mental health practitioner or nurse.
β€’ A direct referral to community services such as Pharmacy First
β€’ A prescription, medical certificate or advice without needing an appointment.
Please note, that it you are booking a regular appointment with the nurse team, eg Chronic Disease reviews or Health checks etc, this can still be done via the telephone.
ℹ️ Why are we doing this?
This change is being introduced across the country to help GP practices manage rising demand safely and fairly. It allows us to prioritise urgent problems while still offering timely care for less urgent issues – and to guide you to the most appropriate service for your needs.
