Why the direction of lymphatic drainage matters

1. Lymph always moves toward regional lymph nodes

The lymphatic system has a one-way flow toward:
• Cervical nodes (neck)
• Axillary nodes (armpits)
• Inguinal nodes (groin)
• Deep central trunks (thoracic duct / right lymphatic duct)

MLD must follow this natural flow so that lymph is actually absorbed into functioning lymph nodes.



2. You must clear the proximal areas first

Before moving fluid from a swollen area, the therapist must:
• First “clear” the lymph nodes and pathways closer to the center of the body
• Then move fluid from the swollen (distal) region toward that cleared area

If done in the wrong order, the fluid has nowhere to go and swelling can worsen.



3. Incorrect direction can increase swelling

Moving lymph distally, sideways, or toward a blocked region can:
• Increase edema
• Cause discomfort
• Slow healing
• Overload compromised lymph nodes



4. Lymph vessels depend on low-pressure flow

The lymphatic system is delicate and has:
• Low internal pressure
• Thin-walled vessels

The correct directional strokes help stimulate:
• Lymphangion activity (the “pumping” of lymph vessels)
• Opening of lymph capillaries
• Movement of fluid into functional lymphatic watersheds

Incorrect direction does not create the right pressure gradient.



5. In lymphedema, direction is essential for rerouting

If lymph nodes are damaged (e.g., after cancer surgery), MLD is used to:
• Redirect lymph away from the affected area
• Move fluid across anatomical lymphatic watersheds into healthy drainage territories

This requires precise directional technique.
