An exciting opportunity has opened within our Assertive Outreach Team, part of our Community Mental Health Service, based in Bridgwater. We’re looking for an enthusiastic practitioner (RMN, OT or Social Worker) who is keen to support people living with serious mental illness (SMI) and complex needs, working collaboratively across a shared caseload.

✅ Full-time (37.5 hrs), Monday–Friday
✅ Flexible start/finish times & condensed hours available
