The healing treatment iastm / guasha  

If you have ever seen an Olympic athletes with dark red circles on their back, you have seen "Cupping." But there is an even older Traditional Chinese Medicine (TCM) technique that looks far more brutal, yet is profoundly effective: Gua Sha (or "Scraping").

In Western physical therapy, it is now known as Instrument-Assisted Soft Tissue Mobilization (IASTM) 

To the uninitiated, it looks like you are severely bruising the skin. But biologically, you are hacking the body's repair system.

Petechiae vs. Bruising

A normal bruise is caused by blunt force trauma that crushes blood vessels, causing deep internal bleeding and tissue damage.

Gua Sha is different. By repeatedly scraping a smooth stone over lubricated skin, you create intentional, highly controlled friction. This pulls stagnant, deoxygenated blood out of the deep muscle capillaries and up to the surface of the skin, creating tiny red or purple dots called Petechiae (in TCM, this is called the "Sha").

You aren't crushing the tissue; you are essentially vacuuming the stagnant metabolic waste to the surface.

The HO-1 Enzyme Miracle

Why purposely create petechiae? Because of the biochemical response.

When your immune system detects these tiny pools of blood at the surface, it panics. It thinks there is a massive injury.

To manage this "fake" injury, your cells massively upregulate an enzyme called Heme Oxygenase-1 (HO-1).

HO-1 is one of the most powerful antioxidant and cytoprotective enzymes in the human body. It breaks down the heme (from the red blood cells) into carbon monoxide and biliverdin, which act as extreme, localized anti-inflammatories.

The Hack: The HO-1 floods the entire area. It doesn't just clear the red marks; it penetrates deep into the muscle fascia, putting out the fire of chronic, decades-old inflammation and tension that a normal massage could never reach.

Fascial Remodeling

Beyond the enzymes, the physical scraping breaks up fascial adhesions—the "glue" that binds muscle fibers together when we sit at desks all day. It physically reorganizes the collagen matrix, restoring immediate mobility and range of motion.

The levator scapulae is a muscle at the back and side of your neck.

It runs from the upper cervical vertebrae (C1-C4) down to the top inner corner of the shoulder blade (scapula).

Function:

Elevates (lifts) the scapula

Helps with neck rotation and lateral bending

Trigger Points & Pain Pattern

Trigger Points:

Tight knots or hyperactive spots in the muscle (often due to stress, poor posture, or repetitive shoulder/neck activity)

Pain Symptoms:

Neck Pain: Stiffness, tightness, difficulty turning the head

Shoulder Pain: Pain at the top inner corner of the shoulder blade

Upper Back Pain: Sometimes radiates along the upper back and shoulder region

Headaches: Pain may radiate to the back or side of the head, mimicking tension headaches

Limited Range of Motion: Turning or tilting the head may worsen pain

Common Causes of Trigger Points:

Poor posture (forward head, rounded shoulders)

Sleeping with poor neck support

Repetitive movements like lifting or carrying heavy objects

Stress and muscle tension

Self-Check

Press along the side of the neck and upper shoulder blade.

Tender spots that refer pain to the shoulder or back of the neck are likely trigger points.

What is fascial release therapy or myofascial release?

Fascial release therapies are known to increase tissue flexibility and joint mobility.

We can help with a technique called Myofascial Release that uses sustained pressure to loosen and lengthen constricted fascia. Cupping therapy is another technique that stretches and lengthen fascia with the use of vacuum cups.

We use IASTM treatment to locate the fasciae restrictions (scar tissue)

Together with engaging your body to heal itself.

Fascial release therapies can help:

break down adhesions between the tissues, softens and re-aligns them,

free up muscles

allow easier and more effective movement

reduce pain,

You don't have to suffer forever with painful ingrowing toenails!

Nail Surgery is a minor surgical procedure performed under local anaesthetic where either part of, or the whole of the nail is removed to prevent ingrowing toe nails and to reduce infection risks and improve life quality.

Not all ingrowing toenails require nail surgery, but if you are suffering it is well worth getting professional help with nail cutting, nail care, and to discuss whether nail surgery is the correct option for you.

I promise it is not as bad as it sounds! I remember having to perform my first nail surgery when qualifying and I was horrified at the thought! Now it is easily my favourite thing to do.

📌 What are trigger points?

This is one of the most common questions I hear in the office. And a very valid one, because trigger points are hypersensitive areas in the muscle that can cause pain - often in a completely different place in the body than their actual location.

🔺 How do they work? For example, a tense muscle in the spine area can cause chest pain. Neck muscles - headaches. Shoulder muscles - elbow or wrist pain.

📸 An example in the photo: the multifidus muscle - trigger points can cause pain both in the back and the front of the body.

🔄 Where do trigger points come from?

• chronic overload
• stress
• lack of movement
• micro-injuries
• poor posture or ergonomics

🧠 And although the problem has been known in physiotherapy for decades (thanks to the work of Dr. Janet Travell and Dr. David Simons), it is still often overlooked in pain diagnostics - even though it can affect up to 90% of us!

💡 That's why it's worth remembering - pain does not always mean a problem in the place that hurts. Sometimes it is the trigger points that are to blame - and working them out (manually or through self-therapy) can bring significant relief.

We had a last minute appointment yesterday.
Our patient had problem sleeping and moving head from long distance driving and stressful work.

After our deep tissue work and IASTM we added myofascial cupping to enhance healing and nourish the fasciae and muscle tissues.
