Today we are celebrating the Transgender Day of Visibility! 🥳🏳️‍⚧️

This day is dedicated to celebrating the accomplishments and victories of transgender people while raising awareness of the work still needed to end the continued discrimination, harassment, and harm against the trans community and their families.

Cheltenham Central PCN have a proud partnership with Identity Space Glos to provide support and advice to our transgender patients. Here is some of the feedback from our attendees:

"I want to have a better relationship with my family. It has hurt me how my brother has just not spoken to me since telling him I was changing my name. Having friends at the group has made me feel valued and it has given me a focus."

"I am really happy attending this group. Sometimes we talk about hard stuff and other times I leave with aching cheeks because there has been a lot of laughter too. I don't notice the negative media quite so much now."

Today we’re celebrating National ! 🥳

A huge thank you to our amazing Social Prescribers who work tirelessly to support patients *beyond* traditional healthcare. We’re incredibly proud of the difference they make every day in helping our patients live healthier, happier lives. 💚

By listening, showing understanding about what matters most to people, and connecting them with local services and community support, they help improve wellbeing, reduce isolation, and empower individuals to take control of their health.

Here is some feedback we wanted to share from a young patient after being supported by us:

‘'I think the top thing is your engagement and communication with me. You come up with solutions and ways to support myself and you give so much honest feedback, this has been so helpful. Out of everyone I’ve ever spoken to, you are the only one who listens and has been helpful in the things you say. You've really helped a lot. I take away a lot from our sessions like help with my anxiety and how I see things. You've had a positive impact on my life. Thank you for everything'.

We're sad to announce that our walking group has come to an end!

Although our regularly attending patients may still enjoy meeting, it will no longer be formally led by our Social Prescribing team.

If you are struggling for food this Christmas, your GP surgery is able to issue you with a foodbank voucher. You need to bring this voucher to the local foodbank to receive a parcel containing 3 days of emergency food.

The opening times for Cheltenham Foodbank over Christmas and the New Year will be:

If you haven't heard - we have a walking group that meets up every fortnight to have a wander around Pittville Park. It's been a lovely activity to get patients outside for a breath of fresh air and brisk exercise. 🚶‍♀️🚶🚶‍♂️

Our group of mixed ages and abilities has built some wonderful friendships over time and we keep each other motivated to live well for longer. Of course, we'd still love to have a few more join in! 🙋‍♀️💪

We meet again tomorrow at 1pm outside Central Cross Cafe.

We delivered another successful Digi Hub this week at the Wilson Health Centre with our lovely new Digi Hub Mentor, Rachel! GlosDigi

Here are some of the highlights:

⚙️ We helped a carer access her husband's medical records. He had lots of appointments and test results and now she can view all of this conveniently on her phone via the NHS app.

⚙️ We showed a patient how to to view all of their prescriptions, as well as just their repeat ones on the NHS app.

⚙️ We helped a patient log in to the NHS app and troubleshooted problems with installing it on their device.

⚙️ We helped a patient update to the latest version of the NHS app and how to check for future updates, making sure they get the best functionality and experience.
