Team Around the Client

At Rebuild 4 Life we build a team around the Client. We co-ordinate with a multi-disciplinary team of professionals who follow the Client through their rehabilitation journey, maximising their potential, and supporting them to manage the long-term effects of their injury to maintain and improve the client’s quality of life, function, independence, social and vocational opportunities. The role of the Rebuild 4 Life case manager includes assessing the client’s needs and supporting clients to recruit and manage a bespoke support package, procure services from various care and therapy providers. We monitor the service provision ensuring quality and cost effectiveness. This is a collaborative process, which involves establishing a therapeutic working relationship with the client and significant others. At Rebuild 4 Life all our Case Managers are Registered Health Professionals and Advanced Registered Practitioner with The British Association of Brain injury and Complex Case Management (BABICM).

Thanks to for offering to sponsor the petting zoo! And to for offering to sponsor a climbing wall... and to and for offers of support. Also to DJ Marc Tribe, The Kraken and Tracey and Andre. We are very thankful to all our supporters and appreciate all the working together for this event. I am also in discussions with a few people about new developments; An Accessible Art Zone and a Wellness Zone. We possibly have a corporate sponsor for the Wellness Zone but if there is anyone out there who knows people who would like to sponsor something or volunteer on the day. Please let us know asap.
