First Contact Physiotherapy

First Contact Physiotherapy Service is a specialist service for patients with Musculoskeletal (MSK) conditions such as muscle / joint / tendons problems allowing patients to be seen by an expert clinician who can assess, diagnose and treat the patient.

Frist contact MSK service can help with patients aged 16 years or over

• Patients with a new MSK condition or an acute flare up of an existing condition in the last 3 months.
• New joint pain or injury
• New soft tissue / muscle / tendon / ligament strains / sprains
• New Spinal pain
• Acute flare up of chronic problems including osteoarthritis
• Minor trauma e.g. sports injury

FCP physiotherapists will contact the patient via telephone in the first instance to discuss the problem. If the patient requires an appointment, they will arrange this.

Do you know as a patient of Leadgate surgery you have access to an adult first contact metal health nurse?

First contact Adult mental health nurse can help with.
• Problems with mood/low mood/depression
• Reduced motivation
• Anxiety
• Worry/Panic
• Stress
• Tearfulness
• Feeling snappy/irritable
• Minor self-harming behaviour

Issues the first contact team are unable to deal with.

• Dementia
• Psychosis (hearing voices/visual hallucinations)
• Medication
• Patients in a mental health crisis
• Patients already open to Secondary Mental Health Services such as:
• Community Mental Health Team
• Access Team
• Early Interventions in Psychosis
• Older Persons Community Mental Health Team
• Peri-natal Team
• Eating Disorders Team

Is your child struggling with their mental health?

Do you know if they are registered as a patient of Leadgate surgery they have access to a young person's first contact mental health nurse?

First Contact young person's mental health nurse can help with.
• Problems with mood/low mood/depression
• Reduced motivation
• Anxiety
• Worry/Panic
• Stress
• Tearfulness
• Feeling snappy/irritable
• Minor self-harming behaviour
• Sleep
• Eating Difficulties
• OCD (Obsessive Compulsive Disorder)
• Psychosis (hearing voices/visual hallucinations)
• Emotional/behavioural difficulties where the child or young person has a need associated with a neurodevelopmental condition, such as autism/social communication needs or attentional difficulties such as attention deficit hyperactivity disorder (ADHD/ADD).

Issues the first contact team are unable to deal with.

• Medication
• In a mental health crisis i.e., actively suicidal are not suitable.
• Nocturnal enuresis
• Post Diagnostic interventions for Autism
• Parenting
• Pre-School without already been seen by universal services
• Counselling
• Bereavement Counselling
• Anger Management

Patients already open to Secondary Mental Health Services such as:
• CAMHS
• Early Intervention Psychosis
• Eating Disorders Team

Would you like to be involved in Leadgate Surgery's patient participation group?

We are looking for expressions of interests from patients who are registered at the surgery, who have a keen interest in the local community and a passion for service improvement.

This will be an opportunity for patients to deliver new ideas and suggestions to the surgery, and for the surgery to share updates and information.
