Following on from our PPG meeting, there are some people who are still unsure on how to access and fill out our online consultation from. Please see below screenshots of how to access the form and the questions it will ask you to fill in. The form is easily accessible from our website, and you will not be asked to upload any identification documents - https://consettmedicalcentre.co.uk/

The NHS app does ask you to confirm your identity and can be difficult to set up – Building Self-Belief CIO host digital inclusion sessions in Consett where they offer free support with the NHS app.

If you do not have access to the internet or can't use a computer or smartphone you can still call reception, and the receptionist will either help you fill in the form or fill in the form for you.

Our Total Triage approach makes it easier for patients to contact us and receive the care they need - quickly and safely.💙

🔔 Total Triage – How to See a GP 🔔

To help us get you the right care as quickly as possible, our practice uses Total Triage for all GP appointments.

👉 If you need to see or speak to a GP, please complete our online consultation form first.
This allows our clinical team to assess your request and make sure you’re directed to the most appropriate care — whether that’s a GP appointment, advice, tests, or another service.

🖥️ Use the online form via our website or this link - https://accurx.nhs.uk/patient-initiated/A83018

📞 If you’re unable to use the internet, our reception team can help you complete the form by phone.

Using Total Triage helps us:
✔️ Prioritise urgent problems
✔️ Reduce waiting times
✔️ Ensure you see the right clinician first time

Thank you for your cooperation and understanding 💙

Please can we ask that if you are able to do so please fill in the online form yourself rather than calling the surgery (A relative/friend can also fill it in on your behalf). This will free up the phone lines for the patients who are unable to fill it in themselves such as the elderly or people without access to the internet.

You can access the form on our website. It is very simple to fill out so please give it a go if you need to contact us. Please include as much information as possible for the GP to triage you appropriately.

Thankyou.

Unfortunately, we have experienced repeated misuse of our toilet facilities. We know it is inconvenient but due to these ongoing issues we will now be keeping them locked. If you need to use them, please ask at reception for the key.

Our priority is to maintain a safe and clean environment for all patients.

Thank you for your understanding and cooperation.

Just a reminder for all of our patients, needles have recently been found in the patient toilets. This is incredibly unsafe, as staff, patients, cleaners, and even children could accidentally get injured.

If you need to dispose of sharps for medical reasons, please always use a proper sharps bin or speak to any staff member, who can direct you safely and confidentially.

If you see any needles in the toilets, please inform a member of staff and do not touch or move them yourself.

Thank you.

Winter Coughs – what you need to know 🌬️❄️

Coughs are very common at this time of year – most are caused by viral infections and will clear up on their own within 2-3 weeks.🕒

👉You don’t usually need antibiotics, as these wont help viral coughs.
👉Rest, keep warm, drink plenty of fluids and try simple remedies such as honey and lemon in warm water (not for children under 1).
👉You can use over the counter medicines if needed – your pharmacist can advise what's best.

💡See your GP if:

Your cough lasts more than 3 weeks
You're coughing up blood
You have a high temperature or feel very unwell
You have chest pain or shortness of breath
