Dear Patients

We are happy to share that earlier this month, NHS Somerset approved our proposal to combine the contracts of the four practices within the South Somerset West Primary Care Network. The practices are now working towards combining contracts in mid-January 2026 and will become known as Symphony South Healthcare. Crewkerne Health Centre – Middle Path GP Surgery Reopens to Patients

Dear Patients and Community,

Following the temporary closure of Crewkerne Health Centre, Middle Path Surgery, to allow for urgent repairs back in September, we are pleased to confirm that the site will reopen on 3 November 2025. All necessary remedial works have now been completed, and we are able to resume normal operations.

We would like to express our sincere thanks to all our patients for their understanding, patience, and support over the past two months.

We would also like to extend our sincere thanks to Crewkerne Community Hospital for their steadfast support throughout this period. Their team has been consistently welcoming, flexible, and supportive of both our patients and staff. Their willingness to accommodate our services at short notice has enabled us to maintain continuity of care and uphold the high standard of service our community rightly expects. We are truly grateful for their collaboration and dedication.

Additionally, we wish to acknowledge the senior executives from NHS Somerset, the ICB, and Symphony Healthcare Services, whose continued guidance and support have been invaluable throughout the last eight weeks.

We look forward to welcoming you back to Middle Path Surgery.

🔊 Temporary Closure Update – Crewkerne Health Centre

Following a number of water leaks, Middle Path Surgery, Crewkerne has had to close temporarily. Contractors and surveyors are on site currently assessing the damage and repair required. As soon as an update on timescale is available for reopening, we will advise.

During this period please be assured that all core services, including staff and clinicians, will continue to operate through West One Surgery, West Street, Crewkerne. There will be no reduction in services, appointments, or clinical capacity during this period.

➡️ From 8 September, there will also be a number of dedicated clinics at Crewkerne Community Hospital.

Patients scheduled for these clinics will be notified directly in advance.
A dedicated Crewkerne Health Centre reception team will be present on site to support and welcome patients.

We kindly ask patients attending the Community Hospital to follow the signage provided and liaise with Crewkerne Health Centre staff on site to ensure a smooth patient journey and to minimise any disruption to the hospital’s regular activity.
