This guide explains how to submit a medical or administrative request to Lane End Medical Group via our website or the NHS App.

1. Submit Your Request
• Via the Website: Go to our website, laneendmedicalgroup.co.uk, and click the "Submit a Request" button/banner on the homepage.
• Via the NHS App: Log into the NHS App. Navigate to the Services section, then select 'Contact your GP about a health problem' (for medical queries) or an equivalent option for admin queries.
• Need 1 to 1 help? Contact the surgery to book an appointment for a 1 to 1 help session if you are unable to use the online form or app.

2. Tell Us What You Need
• Choose your request type (e.g., medical advice, sick note, prescription query).
• Fill in the online form: Answer all the questions in the short form accurately and with as much detail as possible.
• Be specific: Include how long you've had symptoms, their severity, and any treatment you've already tried.
• One problem per form: Please submit only one medical or administrative issue per request to ensure efficient triage.

3. Clinical Assessment (Triage)
• Your completed request is sent directly to the practice.
• A clinician (usually a GP-led triage team) reviews the information to decide the most appropriate next step and urgency.
• Requests are prioritised based on clinical need, not who submitted first.

4. Our Response
• We will contact you to let you know how your request will be managed.
• The outcome could be:
o A phone call from a clinician.
o A self-book link for an appointment (phone or face-to-face).
o A message with advice or further instructions from a GP.
o Direction to another appropriate service (e.g., a local pharmacy).
• Urgent requests are typically dealt with on the day they are received.

The images are reviewed by a qualified radiographer or radiologist, and the report is sent to your GP or referrer.

Reminder: If 28 days pass after your exam and you haven't heard back, please contact your referrer directly.

Protect yourself and your loved ones this season! 🛡️ Book into our Flu & COVID Vaccine Saturday Clinic, October 11th, from 9:00 AM - 1:00 PM.

Getting vaccinated is one of the best ways to stay healthy and prevent the spread of respiratory illnesses in our community.

Who is eligible for the vaccines?

Flu Vaccine:

- All adults aged 65 and over.
- Adults aged 18–64 in a clinical risk group.
- Pregnant women.
- Residents in long-stay residential care homes.
- Carers (including those in receipt of Carer's Allowance or the main carer of an elderly or disabled person).
- Close contacts of immunocompromised individuals.
- Frontline health and social care staff.
- Children aged 2 or 3 years on 31 August 2025 (most school-aged children will be vaccinated at school).

COVID-19 Vaccine:

- All adults aged 75 years and over.
- Residents in care homes for older adults.
- Individuals aged 6 months and over who are immunosuppressed (as defined in the Green Book).

North London Hospice invites you to join us for a cup of tea, a biscuit, and a welcoming space to share your thoughts and feelings after the loss of a loved one.

These cafés are Free, open to all, and no booking is needed. Our caring, trained volunteers are here to listen and support you.

We're inviting all eligible patients to come and get vaccinated at Lane End Medical Group. Staying protected is one of the best things you can do for yourself and our community.

Flu Vaccine Eligibility:

- Children aged 2 and 3.
- Pregnant women.
- Adults aged 65 and over.
- Adults and children (from 6 months to 65 years) with certain long-term health conditions.
- Carers and frontline health and social care workers.
- Household contacts of people with a weakened immune system.

COVID-19 Vaccine Eligibility:

Join us for our next monthly Coffee Time Meet Up!

Our next meetup is on Wednesday, August 20th , from 10:30 a.m. to 12:00 p.m. The topic for this meeting is "Energy Talk", which may be very helpful as we head into the colder months.

The event takes place at the Lane End Medical Centre, located at 2 Penshurst Gardens, Edgware HA8 9GJ. It's free to attend. There's no need to book – just turn up.

Improving Access to Care at Lane End Medical Group

At Lane End Medical Group, we are continually reviewing our services and appointment systems to ensure we provide safe, timely, and effective care.

We recognise that getting an appointment can sometimes be challenging, and like many practices across the country, we are exploring different ways to improve this.

One such model is a Total Triage system. This approach means that every request to contact the surgery begins with an online consultation form. The information you provide helps a senior clinician to review your needs and direct you to the most appropriate professional whether that’s a doctor, nurse, mental health practitioner, physiotherapist, or pharmacist.

For patients who are unable to complete the online form themselves, one of our care coordinators will be available to assist and complete it on their behalf.

The aim is simple: to get you the right care, at the right time, with the right person.

Reviews from other practices already using this system have shown positive outcomes, including quicker access to appropriate care and improved patient satisfaction.
