Some time since my last post - but lots of interesting and challenging problems always come to me.
In the Ebchester clinic, a female pt with an 18 month problem with her right knee and outer to front of shin, following a knee pain that came on for no reason. Her knee was injected - primarily for OA - but her burning pain in the shin area did not change, and she was in pain and couldn't walk much at all. This was missed by various therapists and not even assessed fully.
From detailed assessment she had a superior Tibio-Fibular stiffness with traction of the peroneal nerve at this point leading to her nerve/neural pain. Now after her 4th session she is totally pain free and very happy.
I've had a few bad - but different - back pains recently.
1 - 55 yr old male - really nasty discal pain leading to huge neural pain and reduced function. Initially, possibly needing surgical intervention, BUT once MRI carried out it was agreed to follow a conservative approach. 2 months later 75-80% better.Much reduced pain, muscle power slowly returning and altered sensation in feet reducing and returning to normal. He is now beginning to do more flexibility and strength exercises.
2 - Female aged 36 with nasty pain and a lateral shift - pelvis to one side - which is a common sign of back - and discal - pain. Also reduced muscle power and function. Not happy and unable to work. 4 sessions later within 3 weeks, she is back at work PT to FT, has no shift and hardly any pain until the end of the day and fatigue sets in. Now following a specific exercise programme.
3 -54 year old male, farmer. Back pain of 3-4 weeks duration - good and bad - but suddenly worse pain. No shift but pain with cough and sneeze plus some altered sensation in right leg/foot. 7 weeks later back at work fully though not lifting heavy equipment regularly as yet. Minor intermittent soreness and leg feeling virtually normal. Gradually exercising more and feeling happy.
All back presentations are different and require good detailed movement assessment.
Other interesting presentations include -
1 - Female aged 40 - runner with Patello-Femoral problems
2 - Female aged 65 with chronic OA neck pain and severe restricted movements
3- Male aged 55 - long term mid spinal (Thoracic) into the upper lumbar region pain happening only when out running after 3km!
Happily all are improving with a combination of detailed movement assessment, Deep Oscillation Therapy, Acupuncture both IMS and Traditional Chinese Medicine points, manual therapy, taping and more.
Integrated therapy and good specific exercise structure is essential for successful management with all pains and injuries.
