One of the most important things I help people understand about urge incontinence is this:

It’s often about signalling - not failure.

A lot of people come in saying:
“My bladder just doesn’t work properly anymore.”

And that belief alone creates fear, rushing, and loss of confidence.

So I explain it like this.

Your bladder is a messenger.
Its job is to tell you it’s filling.

With urge incontinence, that message often comes:

too early
too loudly
too often

Not because something is broken -
but because the signalling system has become over-alert.

Here’s a real-life example I hear all the time 👇

Someone can sit through a long film with no problem…
but the moment they pull into their driveway, the urge is suddenly overwhelming.

The bladder hasn’t instantly filled in those last 30 seconds.

What’s happened is the brain has recognised a trigger and turned the signal up.

That’s why urge incontinence is as much about the bladder–brain conversation as it is about the bladder itself.

This is also why we don’t treat everyone the same.

At The Physio Crew, we use questionnaires and detailed assessment to understand:

what type of incontinence you’re dealing with
what triggers your urgency
how strong vs how reactive the system is

Because stress incontinence, urge incontinence, and mixed symptoms need different strategies.

For some people, calming and retraining the signalling is key.
For others, improving the support system around the pelvis and trunk makes a big difference.

That’s where tools like PelviPower can be part of a wider plan — supporting the muscles involved in control while we work on retraining timing, confidence, and habits.

There’s no one-size-fits-all fix.
And there shouldn’t be.

One thing that consistently separates patients who get better from those who stay stuck?

Accountability.

Not motivation.
Not the “perfect” exercise.
Not even pain levels.

Accountability.

The patients who improve tend to:

show up even when it’s inconvenient

do the boring basics consistently

tell the truth when they haven’t done the rehab

take ownership instead of outsourcing responsibility

And here’s the bit we don’t talk about enough 👇
Accountability isn’t pressure — it’s clarity.

When expectations are vague, effort drops.
When responsibility is shared but not defined, rehab stalls.
When patients feel like recovery is something done to them, progress slows.

But when someone understands:

what they’re working towards

why it matters to them

how their actions move the needle

Everything changes.

Accountability creates:

structure in uncertainty

confidence in the process

momentum when motivation fades

It also protects patients from the “all or nothing” trap.

Missed a week?
Didn’t do the exercises perfectly?
That’s not failure — it’s feedback.

My role isn’t to police behaviour.
It’s to support responsibility without judgement.

Because real rehab isn’t about being compliant.
It’s about being engaged.

And engaged patients don’t just recover better —
they leave with skills, confidence, and trust in their body long after discharge.

Accountability isn’t harsh.
It’s empowering.

And when done well, it might be one of the most therapeutic tools we have.

One of the most important things people misunderstand about the spine is this:

The spine is strong because it can move — not because it’s stiff.

I often explain the spine like a building designed for earthquakes.

These buildings aren’t rigid.
They’re flexible.

They’re designed to:

move with the force
absorb load
dissipate energy

If they were completely stiff, they’d crack.

Your spine works in a similar way.

It’s built to:

bend
rotate
adapt to load
respond to movement

When people are in pain, they often try to protect themselves by stiffening and bracing.
That response is understandable — but over time it can make movement feel harder and less efficient.

Movement isn’t the enemy.
It’s part of how the spine stays resilient.

Rehab isn’t about locking the spine down.
It’s about restoring confident, controlled movement so the spine can do what it’s designed to do.

When people understand that their spine is adaptable and robust, fear reduces.

And when fear reduces, movement improves — and that’s often when pain begins to settle too.
