I have listed below the areas of help that I can offer. I have worked in community rehabilitation for fifteen years. Helping residents to regain and maintain their lives after an accident, illness or diagnosis. Community based assessments and interventions for 65+ age group:
• Falls prevention (mobility assessment, grab rails, equipment)
• Home hazard assessments
• Access assessments
• Independent living assessments
• Dementia advice
• Goal & target setting (short / med/ long term intervention plans)
• Advice and signposting regarding equipment, assistive technology and local support
• Referrals to other professionals

You can make a self-referral, a friend/family member/professional can also refer you to me, with your consent. I will need to speak to you over the phone before any visit or assessment takes place. If you are currently waiting to have an Adult Services OT assessment or have complex needs, then please continue this path for their input. Charges
• Initial enquiries (telephone, email, face to face) free of charge.
• An OT assessment is £60 per hour inclusive of travel (within 15 miles of Fleet,
Hampshire). This cost includes a written intervention plan, agreed at the end of the visit
between you and me.
• Telephone or zoom assessment £40
• All further visits are £40 per hour.
• Reviews via telephone or online are free of charge. Can you provide or give advice on any equipment that I might need? I can advise you on any equipment that might be useful for you to have at home and where this can be obtained or purchased and the cost. I will provide a choice of suppliers where possible, including via your Local Authority and the voluntary sector. Professional registration
