Lifeline Homecare have recently been awarded the Discharge to Assess contract and will be the provider of this service in the Mendip area, working in partnership with the NHS.
We are now launching a recruitment drive to build the right team and are now looking for Reablement Care Workers to join and help grow our team
Our specialist team will be delivering the Discharge 2 Assess service within Mendip, in particular the Shepton Mallet, Wells, Street and Glastonbury areas. You will be supporting people after a hospital stay, helping them to regain their independence and make the transition to living at home on their own (or with family), once again.
As a member of our D2A team, you’ll travel to people’s homes to provide a high quality tailored reablement programme that focuses on the client’s goals and outcomes.
About the role:
- Supporting people and helping them relearn skills and regain their independence
- Motivating clients to achieve the goals they have set
- Assisting with personal care, including washing, dressing and toileting
- Daily reporting and working with NHS keyworkers
- A sensitive and tactful approach
- A natural desire to help improve client’s lives
- Patience, empathy and a good sense of humour
- To remain calm and focused, even when under pressure
- A respect for others, friendly personality and a real passion to help people relearn skills
Driving for this role is essential as travelling through the Mendip area will be part of it (Frome, Shepton Mallet, Glastonbury, Street and Wells)
In return, we offer Contract hours (7am - 2pm/4pm - 10pm including alternative weekends), Full training will be given
