Teams Medical Practice Teams Medical Practice Our Practice is highly Patient focused and we have a multidisciplinary approach to patients’ health care.

We aim to deliver the best for our patients by combining the skills of the practice team with other health and social care workers in the community. We provide modern health care facilities and focus on continually improving the health of our patients and residents of the community. The Teams Medical Practice core area is postcode NE8 and NE11 (North of Ravensworth Castle, for example Kibblesworth is outside the practice area). We have an open list and anyone living in the core area is welcome to apply to register with us. Please note we open Monday to Friday 8am to 6pm excluding Bank Holidays.

NHS App drop in sessions at the Teams Medical Practice starting Wednesday 16th April 2025.

Help will be available to get people set back up on the App after the merge with Glenpark or to get patients ready for the switching off of the prescription line on 1st July 2025.

Please attend with your device and access to your emails and we will help as best we can to restore services to you!

Merger update - our systems are finally merged!

We are starting to return back to a normal service. We had over 115 prescription requests which we have processed. However there were a few on the prescription line that we could not make out.

Remember - if you were a repeat dispensing patient, your medication may not be at your pharmacy and may need to be re-requested when due.

We will communicate an NHS app update hopefully early next week on how patients can regain access to that service.

Massive thank you to all who have shown patience during this time.

▪️Why should I book an appointment for cervical screening?▪️
Cervical screening is one of the best ways to protect yourself from cervical cancer.

▪️How does cervical screening prevent cancer?▪️
Cervical screening checks a sample of cells from your cervix for certain types of human papillomavirus (HPV).
These types of HPV can cause abnormal changes to the cells in your cervix and are called "high risk" types of HPV.
If high risk types of HPV are found during screening, the sample of cells is also checked for abnormal cell changes.
If abnormal cells are found, they can be treated so they do not get a chance to turn into cervical cancer.

JOB VACANCY - PCN Social Prescribing Link Worker.

Gateshead Inner West PCN are seeking an experienced, self-motivated, hard-working, and patient focused Social prescribing link worker to join our team.

The post holder will work closely with our GP's and other primary care professionals within the PCN to proactively identify and manage a caseload of patients within the community, making sure that support and care is made available to them and that their complex needs are addressed.

This role is offered on a full-time basis, for 37.5 hours per week, Monday - Friday.
