Another powerful transformation from the Lean On Me Legacy

Just a few weeks ago, we were asked to help with an individual that was isolated, lonely, and stuck in a rut, with little motivation and no real sense of purpose.

So much has been achieved in a short space for someone that was lost in a very big world with no sense of purpose or zest for life.

Yesterday, we took a huge step forward. Together, we got outside, got our hands in the soil, and began bringing their garden back to life 🌱🌸

For the first time in years, they were out of their home, reconnecting with something they once loved. Planting flowers, creating something to care for, and most importantly… rebuilding a reason to get up each day.

This is what care truly means.

It’s not just about washing and dressing. It’s about restoring purpose, building confidence, and achieving goals - side by side, with a helping hand 💛

Our legacy isn’t just words. It’s moments like these.

This month’s team meeting focused on what matters most - delivering safe, high-quality care.

Our team spent time reviewing care plans and risk assessments in detail, making sure every document is clear, accurate, and truly reflects the needs of the people we support.

While we aim to be as paperless as possible, with all care plans available on our app, we still take that extra step. Staff review printed copies together, critique them, and ensure nothing is missed before they are reissued for final client approval. It’s also how we evidence to our regulators that everything has been thoroughly read and understood ✔️

Yes… there were plenty of signatures involved too! ✍️

It’s all part of our commitment to continuous improvement, transparency, and delivering care we can stand behind with confidence. 💙

Last week we held our monthly team meeting was a powerful reminder of why we do what we do 💛

We were honoured to have Hayden join us as a special guest, delivering a truly impactful lived experience presentation. He shared honest insights into what good care looks like, and just as importantly, what poor care feels like, highlighting how Lean On Me continues to stand apart from other services.

His message was clear: to achieve the best outcomes, we must work together, reflect on what has worked well, and be open about what hasn’t. Continuous learning and honest conversations are what drive real change.

Hayden’s passion doesn’t stop there..

He also supports training sessions as a lived experience trainer with the Oliver McGowan team olivermtraining, helping to shape better understanding and practice across the sector.

Tanya has kindly agreed for us to share her achievement, and we couldn’t be prouder of her 💛

Tanya has successfully completed her first week making fantastic progress with her hoarding journey. This is a huge step forward, and it shows just how powerful determination and support can be.

We always say it… small steps make a big difference 🌱

Today included a lovely pamper session, with Tanya enjoying having her hair plaited – a well-deserved moment of self-care with the boss 💆‍♀️

✨ Creating goals is an important part of our success with all of our clients, helping to build structure, motivation, and lasting positive change.

Wow… what a day at both ends 💛

Today, our care staff came together to say goodbye to our dear client Josie, giving her a send-off she truly deserved. It was especially touching to be joined by some of Lean On Me’s departed care staff too, who will always remain a valued part of our Lean On Me family. 💛

A moment filled with reflection, love, and respect for a life well lived.

Later, the day lifted into celebration as we marked an incredible 95th birthday for another one of our wonderful clients 🎉🥂

From heartfelt farewells to joyful toasts, these moments remind us just how special it is to be part of our clients’ lives.

We would like to say a huge thank you to all our Lean On Me clients and their families for always including us in such meaningful occasions — it truly means the world to us 💙

Keeping up with CPD for management is essential in supporting our staff to grow and develop 🌱

Today, Claire, Abi, and Jess are attending our first training session with Skills for Care at the National Star College.

The morning began with a well-deserved tribute to Riki Moody from Gloucester Carers Provider, who sadly passed away recently — a touching reminder of the impact individuals can have across our sector.

A great opportunity to learn, reflect, and continue improving the support we provide every day. We’re excited to bring back new ideas and strengthen our team even further.
