Launched at the end of 2025, the Lincolnshire Personalisation in Action Library brings together real stories from across the county that show what’s possible when care starts with what truly matters to people.

Spanning social care, the NHS, neighbourhood teams, and voluntary and community organisations, each story captures moments of genuine listening, shared decision-making, and meaningful change.

Together, these examples highlight the impact of person-centred, strengths-based practice - stronger relationships, more connected communities, and a kinder, more sustainable system.

We’re looking for people who want to make a difference by having real conversations with others about their relationship with health and care services. No formal interviews. No tick‑boxes. Just genuine stories that help build better relationships between people and services.

What does a Listener do?

👂 Talk with people in your community
👂 Hear their experiences
👂 Share what matters most to them about health, care, and wellbeing

Anyone can be a Listener - professionals, volunteers, or people with lived experience.
