Morning - there is just one story from this week's Rossendale Free Press left to post and it is another of mine, along with three photos taken at the event, all about the seminar on regenerative medicine for clinicians organised by Dr Magdy Aglan at his private clinic in Haslingden Pain Consultants Ltd.

A 43-year-old male patient with a history of back surgery presented to our pain unit with severe right-sided sciatica, specifically S1 nerve root pain radiating to his little toe. The pain was debilitating and unresponsive to conventional treatments. He tried few caudal epidural injections which gave him help but only for a short period each time.

MRI scan confirmed a disc bulge (L5-S1) causing right S1 nerve root compression.

He underwent a precisely guided transforaminal injection (nerve root block) that combined a local anesthetic, steroid and PRP (platelet-rich plasma). This innovative approach targeted immediate pain relief (by the local anesthetic), local inflammation (by the steroid) and long-term tissue healing (via the regenerative effects of PRP).

The result was highly successful. The patient experienced significant immediate pain relief and, crucially, sustained improvement that allowed him to return back to his normal work and daily activities within three months.

No excuse for pain.
A 58-year-old female, she had always been active. Since last year, she began experiencing persistent knee pain that worsened with activity. Simple tasks like climbing stairs or getting up from a chair became more difficult, and she often found herself sitting out of family outings due to discomfort.

Frustrated and concerned about her declining mobility, patient saw her GP who diagnosed osteoarthritis of both knees after examination and X-ray.
Patient tried multiple physical therapy and NSAIDs with limited help.

Patient referred herself to our pain clinic. After full evaluation, our consultant recommended and performed a course of knee injections with PRP (platelet -Rich Plasma) followed by a personal rehabilitation program including low-impact activities.

Few weeks later, patient gradually noticed a significant decrease in her knee pain and she returned to her beloved gardening and now, she enjoys more time with her grandchildren.

No excuse for pain.

45 years old female presented to our clinic with right hip pain. The pain is at the point of the hip. She has been suffering for 2 years, however last 6 months pain has increased and now she climbs stairs with severe pain and great difficulty.
Her hip pain is bad specially at night and in getting up after sitting for sometime. The pain spread to the outer side of her right thigh and she couldn’t sleep on her right side.

X-ray did not show any abnormalities in the spine or the hip joint. Her right leg was slightly shorter than the left, however, using insoles did not help.
Pain killers, exercises and physiotherapy did not improve her pain as well.

Careful examination showed tenderness over the point of the right hip. Our consultant diagnosed right hip bursitis. Hip bursitis is a condition with inflammation/ irritation of the bursa that covers the greater trochanter of the femur bone.
The consultant performed injection of hip bursitis with local anaesthetic and steroids. The injection gave the patient excellent pain relief and now she is back to her normal function and can sleep on her preferred right side.

No excuse for pain

71 years old female presented to our pain clinic with back pain.
She has been suffering for years on and off. Last 2 years her low back pain has progressed and her daily activities have been severely reduced. She became depressed as she was unable to go out with family and friends.

X-ray and MRI scans showed lumbar degenerative changes (spondylosis). She tried multiple treatments including physiotherapy, osteopathy and chiropractic manipulation but with no help. Simple pain killers and homeopathy did not help her back pain, and stronger medication gave her intolerable side effects.

She was told that she had spine wear and tear damage due to aging and nothing could be done.

Our consultant explained that getting older is not a disease. Careful examination identified multiple factors including tenderness over bilateral sacroiliac joints (SIJ) & over facet joints and muscle spasms.
As the worst pain was over SIJ, the consultant performed bilateral SIJ injections with local anaesthetic and steroids. The patient reported >50% improvement in her pain and function. Now she is very satisfied as can enjoy going out and having holiday with family and friends.
