X-PERT programmes are ideal for people who are at risk of developing Type 2 diabetes or living with prediabetes, have Type 1 or Type 2 diabetes (newly diagnosed or established), and anyone overweight or obese looking to lose weight and transform your health. Our education can be accessed via GP Referral, Remote Groups, Digital Apps, Handbooks and Audio Books/Subtitled Videos. Our programmes were c

o-created with patients to ensure they deliver a powerful, rewarding and enjoyable experience. The audit results have clinically demonstrated that our programmes help people lose weight, improve their HbA1c, reduce medications and place conditions such as prediabetes and Type 2 diabetes into remission. We have been delivering self-care diabetes education since 2008 and delighted to work with so many commissioners and have the opportunity to share our knowledge with so many people. Did you know? X-PERT programmes support people with prediabetes, type 1 and type 2 diabetes, and weight management to build the knowledge, skills and confidence to better manage their health.

Programmes are available face-to-face, online, or through the X-PERT Diabetes Digital Programme, which can be accessed on a phone, tablet or computer in 15 languages.

You may be able to access X-PERT free through an NHS referral from your GP or diabetes care team. If a programme is not available in your area, you can also purchase digital access directly.

Struggling to get motivated about your health? You’re not alone 💙

Feeling drained, stressed or stuck is completely understandable. The important thing to remember is that motivation doesn’t have to start with a huge life change. Sometimes even a very small goal can be enough to help you get moving again.

One helpful approach is the X-PERT five-step goal setting model, which encourages you to:
• Choose a goal that is meaningful to you
• Think about any barriers that may get in the way
• Plan how you can reduce or avoid those barriers
• Make sure the goal fits with your lifestyle, feelings and values
• Decide for yourself what feels realistic and right for you

Research shows that the most successful behaviour changes come from internal motivation — doing something because it matters to you, not because someone else thinks you should.

Here are a few ways to build internal motivation:

✨ Be clear about your “why”
What would this change do for you personally? More energy? Better health? Greater confidence?

✨ Be honest with yourself
Tracking your habits, mood, activity or food can help you spot small changes that could make a big difference.

✨ Find enjoyment
You are far more likely to stick with something if you enjoy it, whether that is a new recipe, a walk with a friend or trying a new activity.

✨ Ask for support
Talking to a supportive friend or family member can provide encouragement and give you an extra push to keep going.

Remember, behaviour change can feel awkward at first, but over time new habits can become just as natural as old ones. Start small, stay kind to yourself, and focus on what matters most to you.

Why lifestyle education remains central to weight management care.

Lifestyle education and behaviour change are fundamental across the weight management pathway — supporting long-term outcomes for patients whether they access behavioural support alone, medication, or bariatric surgery.

That’s why Derbyshire Community Health Services NHS Foundation Trust partnered with us to deliver the Weight & Wellbeing programme as part of its Tier 2 weight management offer.

Early outcomes from the first three programmes show:
• 75% programme completion
• Average weight loss of 8.4 kg
• Average BMI reduction of 2.9
• 27.1% improvement in eating behaviour scores

These programmes help provide the knowledge, skills and behavioural tools that underpin long-term weight management — including for patients who may later access specialist services or pharmacological treatment.
