X-PERT Health

X-PERT Health We help transform people’s lives through diabetes, insulin and weight loss education programmes.

X-PERT programmes are ideal for people who are at risk of developing Type 2 diabetes or living with prediabetes, have Type 1 or Type 2 diabetes (newly diagnosed or established), and anyone overweight or obese looking to lose weight and transform your health. Our education can be accessed via GP Referral, Remote Groups, Digital Apps, Handbooks and Audio Books/Subtitled Videos. Our programmes were co-created with patients to ensure they deliver a powerful, rewarding and enjoyable experience. The audit results have clinically demonstrated that our programmes help people lose weight, improve their HbA1c, reduce medications and place conditions such as prediabetes and Type 2 diabetes into remission. We have been delivering self-care diabetes education since 2008 and delighted to work with so many commissioners and have the opportunity to share our knowledge with so many people. Why lifestyle education remains central to weight management care.

Lifestyle education and behaviour change are fundamental across the weight management pathway — supporting long-term outcomes for patients whether they access behavioural support alone, medication, or bariatric surgery.

That’s why Derbyshire Community Health Services NHS Foundation Trust partnered with us to deliver the Weight & Wellbeing programme as part of its Tier 2 weight management offer.

Early outcomes from the first three programmes show:
• 75% programme completion
• Average weight loss of 8.4 kg
• Average BMI reduction of 2.9
• 27.1% improvement in eating behaviour scores

These programmes help provide the knowledge, skills and behavioural tools that underpin long-term weight management — including for patients who may later access specialist services or pharmacological treatment.

Free up GP capacity through structured self-management education. We can implement programmes quickly to assist you and your patients. Contact us at admin@xperthealth.org.uk to arrange a short introductory call.

Social Prescribing Day is an annual awareness day held on 26 March.

At X-PERT Health, we believe people are at the centre of their own health. On Social Prescribing Day, we celebrate how community-connected, education-based approaches help people manage diabetes, improve wellbeing and build healthier lives together.

Social prescribing recognises that many factors affecting health — such as loneliness, lifestyle, confidence and access to support — can’t be addressed by medical treatment alone. By connecting people to education, activities and community resources, it helps individuals take a more active role in managing their health.

Strengthening Tier 2 weight management capacity in Derbyshire.

In partnership with Derbyshire Community Health Services NHS Foundation Trust, we have delivered Weight & Wellbeing programmes to support patients within the Tier 2 stage of the weight management pathway.

With waiting times for specialist Tier 3 services increasing in many areas, strengthening Tier 2 lifestyle education and behaviour change support can help ensure patients access the right level of care at the right time.

Early results from the first programmes delivered in Derbyshire show:
• 75% completion rate
• Average weight loss of 8.4 kg
• Average BMI reduction of 2.9
• 27.1% improvement in eating behaviour scores

Lifestyle education is fundamental across the pathway — supporting patients whether they continue with behavioural support alone, access specialist services, or later receive treatments such as weight loss medication or bariatric surgery.

Programmes like this help systems increase capacity, strengthen pathway adherence, and support long-term behaviour change.

Ever found yourself eating when you’re not physically hungry?
Most of us have — and it’s not a lack of willpower.

There are two distinct experiences we often call “hunger”
👉 Physical hunger – when your body truly needs fuel.
👉 Psychological hunger – food becomes a response to stress, boredom, habit, or even the clock.

Psychological hunger often shows up when:
• We snack automatically
• We eat to unwind
• We crave something specific — even after a meal

A simple pause can make a powerful difference. Next time you reach for food, ask yourself: “What do I actually need right now?”. Sometimes it’s nourishment. Sometimes it’s rest, comfort, connection — or even just water.

Understanding why we eat is just as important as what we eat.
If you’d like practical guidance on recognising and managing psychological eating, our handbooks explore more. Find them in our online shop: Diabetes | Weight & Wellbeing | Low Carb Living
