ICYMI had a great time at IMSH 2022 this week! We were super excited to introduce Simbodies new EMS-Man, our first Emergency Medical Skills Man.

The EMS-Man is a manikin thoughtfully designed for pre-hospital training, featuring over 30 (!!) intended medical procedures: from airway management, IO access and CPR, to bleed control and wound packing. Procedures can be repeatedly performed with minimal maintenance required.

Tension pneumothorax is a life-threatening condition, which, left untreated can be fatal within a matter of minutes. Studies suggest that tension pneumothorax is the second most common cause of preventable death on the battlefied.
Standard clinical management of a suspected tension pneumothorax includes insertion of a cannula into the pleural cavity . When a patient has a tension pneumothorax, needle chest decompression can be life-saving.

There have been widespread reports of complications following needle chest decompression. Commonly used devices, such as intravenous cannula, may not be long enough to reach the pleural space or they may kink and occlude.

There are a number of important aspects to consider when performing a needle decompression for the treatment of tension pneumothorax. These include the size of the individual chest wall, the product chosen to perform the procedure and also how certain the individual is of the tension pneumothorax diagnosis. These three criteria include:
1. Chest wall thickness
2. Decompression failure
3. Diagnosis and safety

When looking for the right solution for to perform needle chest decompression consider the following:
• Needle Length – the needle should be long enough to reach the pleural space in an increasingly overweight civilian population and increasingly muscular military population. Most current devices do not meet this requirement
• Kinking – any product used for needle decompression should be kink resistant. Kinking can cause occlusion and render the device non-functional. Most current devices do not meet this requirement
• Occlusion – Occlusion by blood, tissue or other materials can block the catheter/cannula and render the device non-functional. Addition of fenestrations/holes in the catheter/cannula will enable the device to be functional even if the primary channel is blocked. Most current devices do not meet this requirement
• Air entrainment – air influx into the pleural cavity should be avoided. Attached of a one-way valve and capped product would allow air and fluid out of the pleural space but would not allow any air influx. Most current devices do not have this feature
• Safety tip – the addition of a Veress tipped needle to the device or similar would provide and extra safety dimension to a needle decompression product. Most current devices do not have this feature.

Meet the Medics | Eddy's Story

Meet Eddy Vershilovsky, who started his medical career as a combat medic in 1999 and started working as an EMT Basic for Israeli national EMS. He became a paramedic and continued his EMS duties, where his first mission overseas was a training project in Panama where he had to build an EMS system for Panama City, including paramedic training, dispatch, and ongoing supervision.
Eddy has a master's degree in Emergency Preparedness and Disaster Response, and has worked on medevac aircraft in Israel, and was also deployed to Ukraine during the Crimea crisis.
During peace times, Eddy trains others in tactical casualty care and says of his role, 'One of my favorite things is traveling to places with different cultures and habits and seeing how the language of emergency medicine connects everyone.'

Meet the Medics | Andrea's Story:
Meet Andrea Spivey, an ex-trauma nurse with 17 years of experience in the ER and level 1 trauma centers.
After graduating with a degree in nursing, Andrea participated in twenty-four-hour ride-alongs with fire departments to increase her skills in treating trauma patients at the scene. Andrea then facilitated medical education, which has turned into a dual role and now includes sales.
She describes her favorite part of the job as 'sharing knowledge with care providers to give them confidence and the best tools on the market to save people's lives. There is nothing more humbling and rewarding than to help another human during their time of need.'
