🎄 Christmas at Kew Ward, West Middlesex Hospital 🎄

Over the Christmas period, our team were at Kew Ward speaking with a client’s family to help plan care for a safe and supported return home.

Leah visited Mr C for an assessment, working closely with his family to arrange short-term care ahead of his discharge a few days later. This support was planned for just a few days, providing reassurance until both Mr C and his family felt confident he was settled back at home independently.

We understand that coming home from hospital can be stressful for both the client and their family, particularly during the festive period. Having the right care package in place is crucial for recovery and for helping clients regain the level of independence they had before going into hospital.

Our team carries out timely assessments and matches the correct level of care, ensuring all support is in place for a smooth and confident return home.
