Combe Coastal Practice is a group of doctors who work with a Primary Healthcare Team to provide a complete range of medical services. We are strongly committed to the NHS and pride ourselves in providing a personal, traditional type of family medicine. In view of this, a strong emphasis is placed on health promotion and education by both the GPs and Nursing Team. Our vision is 'to improve and develop all that you value in General Practice, whilst working in a larger business structure in order to operate more effectively in the New NHS'. Fantastic new job opportunity as PCN Pharmacy Care Co-ordinator - The purpose of the role is to support the Primary Care Network Pharmacy Team by coordinating the administrative and operational processes. The role helps to deliver structured medication reviews and medicines optimisation, enabling clinicians to focus on patient care while operational systems run efficiently. You will act as a key coordination point across teams and external partners, supporting quality improvement and excellent service delivery within primary care. A Primary Care Network (PCN) is a group of local GP practices working closely together – and with other healthcare professionals, community services, and the voluntary sector – to provide more coordinated and personalised care for patients.

North Devon Coastal PCN includes 3 GP surgeries and two branch sites. We provide quality NHS healthcare services to around 45,000 patients and aim to ensure patients receive proactive, joined-up care tailored to the needs of our diverse population.

We want your appointment at the practices to be as smoothand supportive as possible. Whether you're coming in for a routine check-up or something more specific, our team is dedicated to providing safe, personalised care.

Here’s a quick guide to help you prepare and know what to expect during your appointment.

Making the Most of Your Time with the GP

We know that appointments can sometimes feel a bit rushed. That’s why we’re using a new tool called Heidi - a secure AI assistant that helps us take notes during your consultation.

How does Heidi work?

Heidi quietly listens during your appointment and creates a written transcript. This allows your GP to focus fully on you and the conversation - not the keyboard. After your appointment, your doctor carefully reviews the transcript to ensure it's accurate and complete.

Your privacy is our priority.

No recordings are stored after the consultation. Heidi doesn’t make any decisions - your GP remains fully in control. If you'd prefer not to use this tool, that’s absolutely fine, just let your GP or nurse know at the start, and we will switch it off.

Our goal is to make every appointment feel calm, respectful, and tailored to your needs. If you have any questions before, during, or after your visit, don’t hesitate to ask. We’re always happy to help.

Preparing for Your Appointment

To help you get the most out of your time with the GP, it’s helpful to come prepared with:

- A clear list of your symptoms, including when they started and whether they’ve changed over time
- Any photos (e.g. of rashes or swelling) that might help us understand the issue
- For high blood pressure concerns: please bring at least a week’s worth of readings, if available
- A list of questions or concerns you’d like to cover

We kindly ask patients to focus on one main issue per appointment. This helps ensure we can give each concern the time and attention it deserves. If you have several issues, we’re happy to arrange follow-up appointments.

One Ilfracombe and the Wellbeing team are running a 6 week healthy living workshop in Ilfracombe. These sessions are for over 18s.

Example of Areas covered:
Making changes, stages of change, goal setting and habit formation.
Healthy eating. Label reading, understanding macro and micro nutrients, understanding energy balance
Physical activity: how much PA should we be doing, monitoring and setting activity targets
Mindfulness, dealing with stress, sleep hygiene
Dealing with relapse/lapse.

🚨 Important Monday Morning Reminder!
Monday mornings are our busiest time of the week. Whilst we have seen a decrease in non-urgent calls on Mondays there are still some that would be better if they were later in the afternoon or later in the week! Please only call Today if your concern is urgent and needs attention today. Routine or non-urgent queries are best handled later in the week or during our quieter afternoon periods. We may ask you to call back if appropriate.
