Individuals with long term conditions, physical disabilities, cognitive impairments, sensory loss.
People recently discharged from hospital.
Stroke survivors and those with acquired brain injury requiring specialist assistance.
The homeless, refugees and asylum seekers, minority ethnic groups.
Anyone who may not fall within the above categories but requires care and support.
