We are new to the area, however we come with a wealth of experience in providing 24 hour care to individuals requiring extra care to support independent living. We intend to provide exceptional care, exceeding Client’s expectations by ensuring we have the highest level of dedicated staff. We intend to create a company culture of complete transparency and in return develop a cohesive team built on trust and mutual respect. Hours will be block in most cases ranging from 4 – 8 hours in one location. There will be an element of personal care but will also include assisting person with household duties, socialising, integration into the community to include leisure activities. Waking night shifts will be available. Excellent rates of pay, permanent contract, 21 days paid holiday, pension, performance related bonus scheme.
Job Description
Job Purpose
To look after the physical, emotional, cultural and social support needs of the Clients using a person centred approach to promote independence and progression towards being as independent as possible, working within a building and in the community supporting clients with additional needs. To observe and promote the Client’s choice, independence, dignity, privacy, fulfilment and other rights through positive engagement and learning activities. Through accurate monitoring and reporting of activities against clients’ objectives and recording all interventions that support the clients’ progression.
To support and develop team members in areas they may need. To actively participate in planning and creating activities and learning for clients. Writing schemes of work and activity plans in line with client objectives. To maintain safe and clean working environments.
To create and maintain good professional relationships with Clients, their family and friends and other stakeholders.
To actively support clients and achieve progressions required by them, in relation employment, education, to housing, finance, and any other areas which lead to a better quality of life, being flexible within the team to support others and clients when asked.
To adhere to all regulatory and statutory obligations and business policies, procedures and guidelines and to positively market projects and promote a positive, personal and professional profile, ensuring the good reputation of Concorde Care in the Community at all times and attract funding opportunities

Job Responsibilities
Support provision
To provide support to Clients with a wide range of needs, illnesses disadvantages, and disabilities and ensure Safeguarding guidelines are in place and understood fully. Safeguarding must be actioned upon as at all times and it is the responsibility of the employee to ensure they fully understand the full remit of Safeguarding.
To know and understand the care, emotional wellbeing, and progression support required for each of the Clients and to ensure this is delivered in a person-centred way, being reviewed quarterly.
To undertake the tasks detailed in the Client’s care and support plan using a person-centred approach and in the least intrusive person-centred way, to promote quality of life if available, if not to work holistically to achieve sustainable outcomes in the client’s best interests. To monitor risk assessments and dynamically update, along with behaviour plans. (if appropriate).
To encourage the independence and motivation of the Client and not foster dependent behaviour encouraging empowerment.
To provide input into the care and support plans of Clients by regularly reviewing and liaising with client and support circle feeding back to the Nominated Manager and staff if appropriate.
To engage clients in activities internal or external, encouraging, educating, supporting and guiding them to progress and achieve the best outcomes for themselves and their family / support circle based on case loading requirements, and to create and manage those activities with clients ensuring schemes of work and activity planners are in place for each activity.
To maintain good communication and develop effective working relationships with Clients, family members, professionals and colleagues.
To help the Client to maintain contact with their family and friends, and encourage new friendships through joint working with clients within the NSC.
To ensure the environment is safe for the Client, whilst respecting the Client’s choice and rights, and ensure that all areas of work are safe and clean ready for clients to attend
Recording and Reporting
To maintain detailed accurate records in respect of all aspects of the activities, projects, schemes of work activity planners and clients’ requirement of service. To maintain reviews and diarise reviews appropriately, and conduct them in a timely manner to a high standard recording information on care systems.
