Our IMPACTAgewell® Officers support Older People 60+ living with long-term health conditions, helping them stay independent, connected, and well in their own homes. We work in partnership with GP surgeries, community pharmacists, and NHSCT social work teams to provide coordinated, person-centred support.

Officers visit you in your home for up to six months, focusing on what matters most to you. This could include:
🏠 Making your home safe and comfortable
❤️ Supporting your health & wellbeing
🤝 Connecting you to community activities and services
🔆 Planning for your future and staying independent

📢 Missed Appointments – Please Help Us Help You

In the last month, 68 appointments were missed at the surgery - the majority of these patients would have received a text message reminder the day before the appointment.

Each missed appointment is time that could have been used to see another patient who may be waiting for care.
When appointments are not attended, it puts additional pressure on our already busy service and delays access for others.

If you can no longer attend, please contact the surgery to cancel as soon as possible so the appointment can be offered to someone else.
If your contact details have changed, please contact the surgery to update these.
