✍️ A Message from the Clinical Director – Celebrating 5 Years of Cross Gates PCN
As Clinical Director of Cross Gates Primary Care Network, I feel incredibly proud to reflect on the journey we’ve taken together over the past five years.
When we developed our network, our shared goal was simple: to work together more closely to deliver high quality, patient centred care for our community.
Together with our four fantastic practices Ashfield, Colton Mill, Family Doctors, and Manston Surgery, we’ve built a PCN that serves over 33,000 patients with compassion, innovation, and dedication.
What makes me proudest is not just what we’ve built, but how we’ve built it through partnership working, great teamwork, and ongoing commitment to local needs.
Our PCN has worked closely with:

These partnerships have enabled us to expand access, provide enhanced and supportive care, and respond to the evolving health and wellbeing needs of our population.
We’ve developed a diverse multidisciplinary team of clinical pharmacists, physio, mental health and dementia nurses, advanced and proactive care nurses, nurse associates, social prescribers, health coaches, care coordinators, and more.
Our Saturday vaccination clinics have protected thousands each season from flu, COVID, pneumonia, RSV, and shingles.
We’re offering extended access clinics, online consultations, contraceptive services, NHS health checks, frailty reviews and much more all delivered closer to home including our new health bus.
We ensure housebound and care home residents get the care they need, with dignity and continuity.
None of this could be possible without the hard work and dedication of our practices, PCN team and wider partners. Our healthcare system is changing and we are ready for the next chapter of innovation, compassion, and care. For now, here are just some of the Services and Teams that we are delivering to all our patients.
