Breast implants: above or below the muscle?

One of the most common questions I’m asked is whether an implant should sit above or below the chest muscle.

The answer isn’t one-size-fits-all.

In this reel, I explain when I’m happy to place implants above the muscle, often allowing for a quicker recovery, and when positioning below the muscle is the better option for long-term balance and support.

Most importantly, choosing the wrong plane for the wrong patient can compromise the overall outcome. Factors such as tissue quality, existing volume, and anatomy all play a role in making the right decision.

This is why careful planning and an individualised approach are key.

Augmentation Mastopexy – breast enlargement with uplift

A lovely patient in her early 40s who, after overcoming difficult personal circumstances, chose to do something for herself. Following an incredible 7 stone weight loss through diet and exercise, she was left with breast ptosis (droopiness), volume loss and loose, stretched skin.

This was addressed with an augmentation mastopexy using 295cc Mentor Siltex implants placed above the muscle, combined with a full uplift (anchor-shaped scar pattern).

At 1 year post-op, her shape remains stable with maintained volume in the cleavage area, and she continues to train harder than anyone I know.

Timeline: pre-op → 1 year post-op
