Lots of changes to digest!

Huge JRCALC updates:

Yesterday there were some significant updates to the JRCALC and the termination of resuscitation guidelines. Here we’ve put some of the key points.

As always, different organisations and Trusts may follow different guidelines, always ensure you keep up to date with your organisations’ guidelines and always work within your scope of practice:

Key points for resuscitation an termination:
• Resuscitation time has been extended to 45 minutes
• Cardiac arrest confirmed by first Ambulance resource on scene
• If resuscitation is started, the first monitored rhythm will determine termination outcome where no ROSC is achieved
• These outcomes vary depending on specific rhythm outcomes following 45 minutes, please ensure to read your guidelines to fully understand
• Best interest decision making now has a larger impact on the overall decision of resuscitation and termination
• Termination of resuscitation and Verification of death are two distinct times
• Time of death can only be confirmed following 5 minutes of continuous asystole
Overview of all updates:
• Guidance revised in termination of resus in adults and children, verification of death in adults and children
• Advanced life support fully revised and updated in line with 2025 ERC and RCUK updates
• Basic life support minor updates in line with 2025 ERC and RCUK updates
• ROSC management updates with further wording around ventilation and fluid management for different arrests aetiologies
• Updated foreign body airway obstruction section
• Fully revised guidelines for emergency tracheostomy and laryngectomy pre-hospital management
• Newborn life support fully updated
• Cardiac arrest during pregnancy fully updated
• New dental emergencies guidance
• New diving emergencies guidance
• Limb trauma guidance updated
• Paracetamol oral presentation updated to new medicines standardization format
• New paragraph added to drowning section around the identification and management of immersion pulmonary oedema
• Bradycardia algorithm updated to include Adrenaline 1:10000 use
• Labour and imminent birth update
• Page for age at birth and post birth to 1 month updated

Most event medical plans work well — until the environment changes.

That’s the uncomfortable truth.

In straightforward settings, access is easy, evacuation is quick and communications are reliable. In those conditions, many providers appear broadly similar.

But move an event into remote terrain, exposed ground or distributed courses, and the problem changes completely.

Response becomes slower.
Extraction becomes more complex.
And the consequences of delay increase.

At that point, the question is no longer just clinical.

It’s operational.

How quickly can you reach the casualty?
How safely can you move them?
What happens if external rescue is delayed?

This is where most event medical models start to break down.

It’s also where planning, capability and equipment start to matter far more than simply having a visible presence.

Peak Medical and Rescue was built around these environments from the outset.

Not adapted to them — designed for them.

If you’re planning an event where terrain, access or delay could materially affect outcome, it’s worth asking a simple question:

Our crew was out on the hill recently providing medical cover for the first ever Focus Rides Peak District Enduro — a brand-new event bringing technical trails, big climbs and even bigger smiles to the Peaks.

From the first stage drop-ins to the final finishers, our medics were out across the course keeping riders safe. Huge respect to everyone who sent it today!
