• Long‑term condition reviews (asthma, COPD, diabetes, HTN)
• Health assessments & health promotion
• Working as part of a supportive, multidisciplinary team

• NMC‑registered nurse
• Primary care experience OR strong transferable skills
• Great communication & a patient‑centred approach
• Desire to keep learning and developing
